İğneleme ve Konjonktiva Altı 5-Fluorourasil Enjeksiyonu Sonrası Hipotoni ve Ciddi Toksik Reaksiyon Gelişen Bir Olgu

A Severe Toxic Reaction and Hypotonia Secondary to Needling and Subconjonctival Injection of 5-Fluorouracil: a Case Report

Özet

Amaç: Sağ gözünde ağrı ile birlikte ani görme kaybı nedeniyle acil servise başvuran olgunun yapılan ilk muayenesinde görme P+P+ olup göz içi basıncı (GİB) 6 mmHg idi. Kornea hafif ödemli, keratik strialar mevcuttu. Ön kamara sığ, pupil alanında yoğun pigmente membran vardı. Olguya 3 gün önce iğneleme ile birlikte konjonktiva altı 5-Fluorourasil (5-FU) enjeksiyonu yapıldığı öğrenildi. Olgu, 5-FU ya bağlı toksik reaksiyon olarak kabul edilip tedavi için hastaneye yatırıldı. Bu yazıda hipotoni ve ciddi toksik reaksiyonu olan bir olgu ile birlikte iğneleme ve 5-FU enjeksiyonuna bağlı gelişebilecek komplikasyonlar tartışılmıştır

Abstract

Purpose: A patient was admitted to the emergency department with a sudden loss of vision and pain in the right eye. Visual acuity was P+P+ and intraocular pressure (IOP) was 6 mmHg at the initial examination. There was a mild corneal edema and keratic striate in the cornea. The anterior chamber was shallow and dense pigmented membrane was observed on the pupillary area. Needling and subconjunctival injection of 5-Fluorouracil (5-FU) was performed on the patient 3 days before. The patient was found to have a toxic reaction related with 5-FU, and hospitalized for the treatment. In this article, a case with hypotonia and severe toxic reaction and complications related with needling and subconjunctival injection of 5-FU were discussed
