Özet

Giriş: Nontifoidal Salmonella'lar (NTS) tüm dünyada yaygın, çoğu kez gastroenteritle seyreden besin kaynaklı etkenlerdir. NTS türleri genellikle kendini sınırlayan infeksiyonlar yapar ve antibiyotik tedavisi gerektirmez. Bununla birlikte immünsüpresif hastalarda veya hayatı tehdit eden infeksiyonlarda tedavi önerilmektedir. Antimikrobiyallere dirençli suşlardaki artış tedaviye yanıtı olumsuz yönde etkilemektedir. Çalışmamızın amacı, hastanemizde 7996-2006 yılları arasında izole edilen NTS suşlarının sıklığındaki ve antibiyotik duyarlılığındaki değişimi belirlemektir. Materyal ve Metod: Hastanemizde 7 996-2006 yılları arasında izole edilen 512 NTS susunun sıklık ve antimikrobiyal duyarlılıklarındaki değişiklikleri göstermek için çalışma üç döneme ayrıldı. Birinci dönem 7 996- 7 998, ikinci dönem 1999-2001, üçüncü dönem ise 2004-2006 yıllarını kapsamaktaydı. Salmonella suşları standart biyokimyasal özelliklerine göre tanımlandı ve Kauffman White sınıflandırmasına göre spesifik hücre duvarı O ve flajellar H antiserumları (Difco, USA) kullanılarak lam aglutinasyon testiyle serotiplendirildi. Antibiyotik duyarlılık testleri disk difüzyon yöntemi ile "Clinical and Laboratory Standards Institute (CLSI)" önerilerine göre yapıldı. Bulgular: İzole edilen Salmonella enteritidis, Salmonella typhimurium ve diğer NTS'lerin oranları; birinci dönemde sırasıyla %74.2, %18.2 ve %7.6, ikinci dönemde %51.6, %29.0 ve %19.4, üçüncü dönemde ise %48.3, %19.1 ve %32.6 idi. Ampisiline direnç oranı birinci, ikinci ve üçüncü dönemde sırasıyla S. enteritidis için %33.0, 9647.0 ve %6.9, S. typhimurium için %31.6, %42.3 ve %23.5 ve diğer NTS'ler için %7 9, %23 ve%l3.8 olarak saptanmıştır. Trimetoprim-sülfametoksazol direnç oranları ise birinci, ikinci ve üçüncü dö¬nemde sırasıyla S. enteritidis için %12.2, %17.0 ve %9.3, S. typhimurium için %16.6, %19.2 ve %11.8 ve diğer NTS'ler için %8.0, %15.0 ve %13.8 olarak hesaplanmıştır. İkinci dönemdeki S. typhimurium ve S. enteritidis türleri dışındaki NTS grubunda bir suş hariç bir ve ikinci dönemde siprofloksasine direnç tespit edilmedi. Üçüncü dönemde ise siprofloksasine direnç oranları S. enteritidis için %9.3, S. typhimurium için %11.8 ve diğer NTS'ler için %6.9 olarak hesaplanmıştır. Sonuç: Çalışmamızda, S. enteritidis en sık izole edilen NTS susu olmuş ve ülkemizde su sanitasyonunun iyileşmesi nedeniyle, suyla bulaşan suşlardan besinle bulaşan suşlara kayma olduğu düşünülmüştür. NTS suşlarmdaki ilk iki dönemde görülen ampisilin ve trimetoprim-sülfametoksazol direncindeki yükseklik daha sonra yerini azalmaya ve siprofloksasin direncinde artışa bırakmıştır. Bu durumun ampirik tedavide son yıllarda siprofloksasin seçiminde artış olmasına bağlı olduğu düşünülmüştür.

Abstract

Introduction: Nontyphoidal salmonellas (NTS) are common worldwide food-borne pathogens that cause mostly gastroenteritis. NTS often cause self-limited infections and do not require antibiotic therapy. However, in immunocompromised patients or in cases of serious infection, therapy is recommended. The increase in antimicrobial-resistant strains negatively influences response to therapy. The object of our study was to evaluate changes in the frequency and antimicrobial susceptibility of NTS strains isolated between 1996 and 2006. Materials and Methods: In order to examine isolation frequency and antimicrobial susceptibility changes of 512 NTS strains isolated in our hospital between 1 996 and 2006, the study was divided into three time periods, with the first period covering 1996-1998, the second 1999-2001 and third 2004-2006. Salmonella isolates were identified by standard biochemical tests, and serotyping was done according to Kauffman-White classification scheme by slide agglutination test with specific antisera for cell wall O and flagellar H antigen (Difco, USA). For antimicrobial susceptibility testing, the disk diffusion method recommended by the CLSI was used. Results: The isolation rates of Salmonella enteritidis, Salmonella typhimurium and the other NTS were 74.2%, 18.2% and 7.6%, respectively, in the first period; 51.6%, 29.0% and 19.4%, respectively, in the second period; and 48.3%, 19.10% and 32.58%, respectively, in the third period. In first, second and third periods, ampicillin resistance rates for S. enteritidis were 33.0%, 41.0% and 6.98%, respectively, for S. typhimurium were 31.6%, 42.3% and 23.53%, respectively, and for other NTS strains were 17.0%, 19.0% and 13.79%, respectively. Trimethoprim-sulfamethoxazole resistance rates for S. enteritidis were 12.2%, 17.0% and 9.3%, for S. typhimurium were 16.6%, 19.2% and 11.76%, and for other NTS strains were 8.0%, 15.0% and 13.79% in the first, second and third periods, respectively. There was only one strain resistant to ciprofloxacin during the first and second periods. In third period, ciprofloxacin resistance rates were 9.3% for S. enteritidis, 11.76% for S. typhimurium and 6.90% for other NTS. Conclusion: In our study, S. enteritidis was the most frequently isolated NTS. In our opinion, given the better water sanitation facilities in our country, there is a tendency for it to be isolated more from food-borne infection than water-borne infection. In the first two periods, increased resistance rates to ampicillin and trimethoprim-sulfamethoxazole were observed, but in the last period these resistance rates were decreased, and resistance to ciprofloxacin was increased. This was probably caused by the increased prescription of ciprofloxacin as empirical therapy in recent years.
