Özet

Çocukluk döneminde görülen ishal etkenlerinin başında yer alan rotavirüs, mide-bağırsak sisteminde sınırlı kalan enfeksiyon olarak bilinmektedir. Ancak burada rotavirüs ishali sırasında bağırsakdışı artsorunlar gelişen olgu sunulmuştur. Ateşli nöbet geçirme şikayeti ile getirilen beş yaşındaki hastanın aynı zamanda kusma ve ishal şikayetlerinin de olduğu öğrenildi. Genel durumu kötü, bilinci kapalı, ağır sıvı kaybı olan hastanın laboratuvar incelemelerinde asidoz, böbrek yetmezliği; koagülasyon testlerinde bozulma saptandı. Mide-bağırsak kanaması gelişen hastanın izleminde, karaciğer enzimlerinde ve serum amonyak düzeyinde yükselme, kan şekerinde ve beyaz küre sayısında düşüklük saptandı. Dışkı incelemesinde rotavirüs antijeni pozitif bulundu. Erken dönemde başlanan antibiyotik, sıvı, taze donmuş plazma ve trombosit desteği ile genel durumu düzelen hasta taburcu edildi. Rotavirüs ishalli hastaların gelişebilecek bağırsakdışı artsorunlar açısından izlenmesi ve erken dönemde uygun tedavi verilmesi yaşam kurtarıcı olabilir. (J Pediatr Inf 2012; 6: 115-7)

Abstract

Rotavirus is one of the leading causes of severe gastroenteritis in childhood, which is an infection known to be limited to the gastrointestinal system. However, a case of extra-intestinal complication developed during rotavirus gastroenteritis is presented here. A five years old patient with febrile convulsion, also having complaints of vomiting, diarrhea, and fever was brought to our hospital. The general condition of the patient was poor; she was unconscious and severely dehydrated. Her laboratory findings showed acidosis and renal failure; coagulation tests showed impairment. During her monitoring, gastrointestinal bleeding, increase in liver function test values, hyperammonemia, neutropenia, and hypoglycemia developed. The rotavirus antigen was found to be positive in the gaita test. The patient’s general condition improved with administratin of antibiotics, liquids, fresh frozen plasma, and thrombocytes in the early stages, after which she was discharged. Close monitoring of patients with rotavirus gastroenteritis in terms of extra-intestinal complications and applying appropriate treatment in the early stages may save lives.
