Genişlemiş spektrumlu beta-laktamaz (GSBL) enzimleri Enterobacteriaceae türleri tarafından üretilirler ve penisilinler, sefalosporinler ve monobaktamların çoğunluğunu inaktive ederler. îzolatların GSBL üretiminin olup olmadığının belirlenmesi hem hastanın antibiyotik tedavisinin planlanması hem de hastane direnç paterninin bilinmesi açısından önemlidir. Bu çalışmada; hastanede yatan ve poliklinik hastalarından izole edilen Escherichia coli ve Klebsiella izolatlannın GSBL oluşturma oranları ve GSBL pozitif suşlarm çeşitli antibiyotiklere duyarlılıkları araştırılmıştır. Laboratuvanmızda izole edilen 514 E.coli susunun 94 (% 18.3)'ünün, 175 K.pneumoniae susunun 52 (% 29.7)'sinin ve 24 K.oxytoca susunun 1 (% 4.2) 'inin GSBL oluşturduğu saptanmıştır. GSBL oluşturmanın; klinik hastalardan izole edilen E.coli suşlannda % 29, K. pneumoniae'de % 35.8, K.oxytoca'da % 6.7 ve poliklinik hastalarından izole edilen E.coli suşlannda % 7.7, K.pneumoniae'de % 15.4 oranında pozitif olduğu görülmüştür. Klinikte yatan hastalardan izole edilen suşların daha yüksek oranda GSBL oluşturması E.coli için de (p<0.001), K.pneumoniae için de (p<0.01) anlamlı bulunmuştur. GSBL oluşturan izolatlar GSBL oluşturmayanlara göre birçok antibiyotik için anlamlı derecede daha yüksek oranda dirençli bulunmuştur. GSBL oluşturan izolatlann diğer antibiyotiklere duyarlılıkları incelendiğinde iki tür için de en etkili antibiyotik olarak karbapenemler belirlenmiştir.
Abstract
Extended-spectrum beta-lactamase (ESBL) enzymes are produced by members of Enterobacteriaceae family and inactivate the most of the penicillins, cephalosporins and monobactams. Determination of ESBL production in clinical isolates is important in the planning of antibiotic therapy of patient and the survey of antibiotic resistance pattern within the hospital. In this study, prevalence of ESBL production of Escherichia coli and Klebsiella pneumoniae strains isolated from outpatients and inpatients and, susceptibility of ESBL positive and negative strains to various antibiotics were examined. Ninety-four (18.3 %) strains of 514 E.coli, 52 (29.7 %) strains of 175 K.pneumoniae and 1 (4.2 %) strain of 24 K.oxytoca were found to be ESBL producers. ESBL activity was detected in 29 % of E.coli, 35.8 % ofK. pneumoniae and 6.7 % of K.oxytoca strains from inpatients, and in 7.7 % of E.coli, 15.4 % of K.pneumoniae isolates from outpatients. ESBL production was found significantly higher in clinical isolates of E.coli (p<0.001) and K.pneumoniae (p<0.01). The higher rates of resistance were found in ESBL positive strains than in ESBL negative strains and the difference were significant for most of the antibiotics. When the susceptibility of ESBL producing isolates to various antibiotics were examined, carbapenems were found to be the most effective antibiotics in both species.
