Özet

Amaç: Diyabetik hastalarda gerçekleştirdiğimiz diz üstü femoro-popliteal (F-P) bypass girişimleri sonrası hiperglisemi regülasyonunun morbidite üzerine etkilerini incelemeyi amaçladık. Materyal ve Metod: İzmir Alsancak Devlet Hastanesi Kalp ve Damar Cerrahisi Kliniğinde Ocak 1997 — Aralık 2002 tarihleri arasında 39 diyabetik olguya diz üstü F-P bypass uyguladık. Tüm hastalara otojeıı safen ven grefti kullanıldı. İntraoperatif ve postoperatif intravenöz insülin infüzyonu pozolojilenimi Portland Protokolü gereğince yapılan 20 olguyu Grup I ve Portland Protokolüne rağmen kan şekeri 200 mg/dl üzerinde olduğundan IV bolüs insülin takviyeleri uygulanarak 150-200 mg/dl ortalamasına sahip 19 olguyu kapsayan Grup II olarak sınıfladık. Grup I'de kan şekeri düzeyi ortalaması protokole rağmen 200 mg/dl üzerinde saptandı. Bulgular: Ameliyat sonrası ilk 30 günü erken dönem kabul ederek serimizde bu döneme ait Grup I'de 4 olguda yüzeyel yara yeri infeksiyonu ve 2 olguda da femoral bölgede seroma komplikasyonu gelişirken Grup II için bu morbidite faktörlerine maruz kalan olguların sayısı birer tane olup anlamlı düşüklük saptandı. Sonuç: Her iki grupta cerrahi yara yeri infeksiyonu açısından risk faktörleri eşitlendiğinde, gereğinde IV bolus insülin takviyeleri ile intravenöz insülin infüzyonu uygu-lanımının iııeksiyon insidansını azaltacağı düşüncesindeyiz

Abstract

Objective: The aim is to examine the effects of hyperglycemia regulation on the morbidity of diabetic patients after above knee femoro-popliteal bypass (F-P) surgery. Material and method: Thirtynine diabetic cases underwent above knee F-P bypass surgery from January, 1997 to December 2002 in the Cardiovascular Surgery Clinic Alsancak State Hospital, Izmir. In every patient, autogenous saphenous vein graft was used. The patients were classified into two groups according to the posology of intraoperative and postoperative intravenous insuline infusion. The posology of the first group consisting of 20 patients were regulated in consistency with the Portland Protocol. In the second group including 19 patients, when blood glucose exceeded 200 mg/dl. in spite of the Portland Protocol intravenous bolus insuline was administered to maintain an average blood glucose of 150-200 mg/dl. The mean blood glucose level was above 200 mg/dl in group I in spite of the protocole.Results: Accepting postoperative 30 days as the early period, four superficial wound infections and two seromas in th e femoral area, were observed in group I. The number of cases subjected to these morbidity factors was significantly lower, being one and one patient respectively in group II. Conclusion: When the risk factors for postoperative wound infection are equalized among two groups, we believe that administration of intravenous insuline infusion, along with additional IV bolus insuline in case of necessity, will lower the incidence of infection.
