Özet

Amaç: Overin borderline müsinöz tümörlerini invaziv müsinöz karsinomlardan ayırmak için kullanılan histolojik kriterler çok tartışmalıdır. Bu çabşmada, laboratuarımızda tanı konulan överin 26 borderline müsinöz tümörü Dünya Sağlık Örgütünün 2000 yılı kriterleri kullanılarak tekrar değerlendirilip intestinal ve endoservikal benzeri alt gruplara ayrılmıştır. Gereç ve yöntem: Ocak 1996 - Mart 2001 tarihleri arasında incelenen tüm müsinöz tümörler bilgisayar kayıtlarından saptandı. Borderline Müsinöz Over Tümörü tanısı konulan 26 olgunun hematoksilen- eozin boyalı preparatları yeniden incelendi. Bulgular: Överin borderline müsinöz tümörleri, tüm müsinöz tümörlerin %16,8'ini oluşturmaktadır. 26 olgumuzun 23'ü (%88,4) intestinal tip, 3'ü (%11,6) endoservikal tip olarak belirlenmiştir. Intestinal tip tümörlerin 17'si (%73,9) epitelyal atipili, 4'ü (17,3) intra epitelyal karsinomlu, 2'si (%8,6) mikroinvaziv özelliktedir. Psödomiksoma peritonei, 3 intestinal tip borderline müsinöz tümörlü olguda izlenmiş, endoservikal benzeri tipte ise saptanmamıştır. Överde endometriozis endoservikal benzeri tip 1 olguda saptanmış, intestinal tipte ise izlenmemiştir. Olguların ortalama 34,5 ay takip edilebilen 21'si hastalık bulgusu olmaksızın hayattadırlar. Sonuç: Bu çabşmada Borderbne Müsinöz Tümörler güncel tanı kriterleri kullanılarak yeniden gözden geçirildi ve izleme bilgileri ile birbkte histolojik subtiplerin tedaviyi yönlendirmede önemi olmadığı saptandı.

Abstract

Objective: Histologic criteria for the distinction of ovarian mucinous borderline tumors from invasive mucinous carcinomas are controversial. In this study, 26 mucinous borderbne tumors were reviewed and subclassified into intestinal and endocervical like subtype by using World Health Organisation-2000 criteria. Material and method: All mucinous tumors examined in our pathology laboratory during the time period of 1996 until March 2001 were reviewed. The painted hematoxileneosin preparations of 26 cases having the diagnosis of Borderline Mucinous Tumors were reexamined. Results: Borderbne ovarian tumors" constitues 16.8% of all mucinous tumors. Twenty three of our 26 cases (88.4%) were intestinal type, 3 cases (11.6%) were endocervical bke type. Of the intestinal type, 17 tumors (73.9%) were with epithebal atypia, 4 tumors (17.3%) were with intraepithebal carcinoma, 2 tumors (8.6 %) were microinvasive.Pseudomyxoma peritonei was observed in three cases with intestinal type mucinous borderline tumors but this finding was not exhibited in endocervical like type tumors. Endometriosis in the ovary was observed in one case of endocervical like type, but was not seen in any case of intestinal type.Twenty one out of 26 cases are alive with no evidence of disease at an average of 34,5 months of follow up. Conclusion: In this study Borderline Mucinous Tumors were reviewed using updated diagnosis criteria and the unimportance of histologic subtypes together with the observation information, in directing the treatment was determined.
