Gelişimsel Kalça Displazili Çocuklarda Tedavi Yaşının ve Cerrahi Yöntemin Klinik ve Radyolojik Sonuçlara Etkisi: Karşılaştırmalı Bir Çalışma

Özet

Bu çalışmada tedavisine 18 aylıktan önce ve sonra başlanan Gelişimsel Kalça Displazili (GKD) çocuklarda; yaşın ve uygulanan tedavinin, klinik ve radyolojik sonuçlar üzerine etkisini araştırdık.Yöntemler: Ocak 1997-Aralık 2010 yılları arasındaki tipik GKD tanısı konularak, tedavi ve takipleri yapılan hastalardan, en az dört yıl düzenli takip edilen ve son kontrolde beş yaş ve üzerinde olan; 35 hastanın 46 kalçası çalışmaya dahil edildi. Grup 1: 18 aylık, anterior açık redüksiyonla birlikte Salter innominate osteotomi yapılan olgulardan oluşuyordu.Bulgular: Klinik ve radyolojik değerlendirme bakımından Grup 1'de başarılı sonuçlar Grup 2'ye göre daha fazla olmasına rağmen gruplar arasında anlamlı istatistiksel fark yoktu (sırasıyla, p=0,332 ve p=0,425). Diğer yandan revizyon cerrahisi gerekliliği ve avasküler nekroz görülmesi Grup 2'de Grup 1'e göre daha fazlaydı ve istatistiksel bakımdan anlamlı fark vardı (sırasıyla, p=0,30 ve p=0,046). Sonuç: Klinik ve radyolojik başarılı sonuçlar bakımından Grup 1'deki oranların daha yüksek olması dikkat çekicidir. Ayrıca avasküler nekroz gelişimi ve uygulanan ikincil cerrahiler açısından Grup 1 lehine anlamlı fark olması, GKD tedavisinde erken yaşta uygulanan daha basit girişimlerle daha başarılı sonuçlar alındığını göstermektedir

Abstract

We investigated the effect of age and surgery procedure on clinical and radiological outcomes in children with developmental dysplasia of the hip (DDH) who were treated before or after 18 months of age.Methods: Between January 1997 and December 2010, 46 hips of 35 patients were included in this retrospective study. The patients who treated for DDH, followed regularly for at least 4 years and at the last 5 years and above in time of the last controlled. Group 1 (<=18 months) comprised patients who underwent open reduction through the anterior approach, and Group 2 (>18 months) comprised patients who underwent anterior open reduction plus Salter innominate osteotomy.Results: With respect to clinical and radiological outcomes, although Group 1 was more successful than Group 2, there was no significant difference between the groups (p=0.332 and p=0.425, respectively). In contrast, the necessity of a revision surgery and avascular necrosis of Group 2 from Group 1 was higher, and there was a significant difference between the groups (p=0.30 and p=0.046, respectively).Conclusion: Successful in terms of clinical and radiological results to be higher than the rate in group 1 is remarkable. Moreover, avascular necrosis development and implementation of secondary surgery were less observed in young children who were treated with only open surgery, thereby suggesting that better results are achieved with simple procedures and at an early age in DDH treatment
