Özet

Replantasyon ve flep cerrahisinde venöz yetmezlik önemli bir problemdir. Bu çalışmada, sülüklerin eksternal dekontaminasyonu ile kombine infeksiyon profilaksi protokollerinin karşılaştırılması amaçlanmıştır. Sülük uygulama ortalama 3 gün sürmüş, sülük uygulaması ile başlayan antibiyotik profilaksisi 7 güne tamamlanmıştır. Replantasyon ya da lokal flep uygulanan 20 hasta rastgele beşer hastalık 4 gruba ayrılmış, 1. ve 3. gruptaki hastalara transportasyon sıvılarına 12.5 ppm sodyum hipoklorit ilavesi ile 10 dakika eksternal dekontaminasyon yapılmış sülükler uygulanmış; 1. ve 2. grup hastalara profilaktik 2x1 g/gün intravenöz sefazolin sodyum, 3. ve 4. grup hastalara ise 1x2 g/gün intravenöz seftriakson verilmiştir. Hastalar; hastanede kalım süreleri ve sonrasında 14 gün boyunca replantasyon ve flep sağ kalımı ile sistemik ya da lokal infeksiyon açısından takip edilmiştir. Eksternal dekontaminasyon yapılmayan sülüklerin uygulandığı 10 hastanın 6’sında, dekontaminasyon yapılan sülüklerin uygulandığı 10 hastanın 4’ünde; profilaktik sefazolin sodyum kullanılan 10 hastanın 7’sinde, seftriakson kullanılan 10 hastanın 3’ünde lokal infeksiyon gelişmiş, gruplar arasında lokal infeksiyon oluşmasında anlamlı bir fark saptanmamıştır. Dekontaminasyon yapılmayan sülükler ve sefazolin sodyum kullanılan 1.grupta 4 hastada lokal infeksiyon görülürken, dekontaminasyon yapılan sülükler ve seftriakson kullanılan 4. grupta sadece 1 hastada lokal infeksiyon görülmüş ve bu gruplar arasında sınırlı bir anlamlılık saptanmıştır (p=0.058). Anlamlı sonuç alınmaması gruplardaki hasta sayılarının küçüklüğüne bağlanarak çalışmanın sürdürülmesine karar verilmiştir. Hiçbir hastada sistemik infeksiyon görülmemiştir.

Abstract

Venous congestion is a significant problem in replantation and flap surgery. This study aims to compare the external decontamination of leeches and combined infection prophylaxis protocols. The mean duration of leech application was three days, and antibiotic prophylaxis was continued for seven days. Twenty patients who underwent replantation or local flap procedures were randomly placed into 4 groups, with 5 patients in each. Patients in groups 1 and 3 were treated with leeches that were externally decontaminated by the addition of 12.5 ppm sodium hypochloride to transportation fluid for 10 minutes; patients in groups 1 and 2 were treated with prophylactic cefazolin sodium 2x1g intravenously; patients in groups 3 and 4 received intravenous ceftriaxone 1x2 g, daily. The patients were followed up for 14 days with respect to success of replantation and flap survival, and presence of local or systemic infection. Local infection rates were as follows: 6 out of 10 patients treated with leeches which were not decontaminated, 4 patients out of 10 treated with externally decontaminated leeches, 7 patients out of 10 treated with prophylactic cefazolin sodium, and 3 patients out of 10 treated with ceftriaxone. Infection rates among the groups did not show a statistically significant difference. There were 4 local infections in group 1, which was treated with non-decontaminated leeches and cefazolin sodium, and only 1 local infection in group 4, which was treated with decontaminated leeches and ceftriaxone. The difference in infection rates of these groups showed limited significance (p=0.058). The lack of significance was attributed to the limited numbers of patients in each group, and therefore it was decided to continue the study. None of the patients developed a systemic infection.
