Özet
Psödoexfoliasyon sendromu, intraoküler ve ektraoküler dokularda ekstrasellüler mikrofibriler materyalin yapımı ve depolanması ile karekterize yaşa bağlı kompleks bir hastalık olarak tanımlanabilir. Tüm dünyada açık açılı glokomların en önemli nedenidir. Etkilediği dokulardan biride korneadır. Korneada kuru göz, kornea duyarlılık azalması, kornea incelmesi, korneal histereziste azalma, pseudoexfoliatif keratopati gibi klinik bulgularla birliktedir. Kornea endotelde psödoeksfoliasyon materyali ve pigmentasyon birikir. İnvivo konfokal mikroskopik çalışmalarda kornea subbazal sinir dansitesinde azalma, tortiosite artışı , stromal keratosit kaybı endotel hücre sayısında azalma, pleomorfizm ve polimegatizm saptanmıştır. Kornea biomekanik özellikleri değerlendirildiğinde hysteresis ve kornea resistans faktörde azalma saptanmıştır. Tüm psödoeksfoliasyon'lu olgularda, özellikle glokom ve katarakt varlığında korneal tutulum akılda tutulmalı ve korneal hücre dansitesi ve subbazal sinir pleksusu dikkatli değerlendirilmelidir. Gelecekteki çalışmalar korneal tutulumun ciddiyeti ve pseudoexfoliasyonun sınıflandırmasının yapılması konusunda aydınlatıcı olabilir.
Abstract
Pseudoeksfoliation syndrome (PES) is a common age-related systemic extracellular matrix disorder characterized by the production and accumulation of abnormal fibrillar material in ocular and nonocular tissues. It is the most common recognizable cause of open-angle glaucoma worldwide. Clinical signs of cornea with PES include decreased corneal sensitivity, thinning of the central corneal thickness, and impaired tear film stability and dry eye, decreased corneal hysteresis, pseudoexfoliative keratopathy. deposition of pseudoexfoliation material and pigment on the corneal endothelium.In vivo confocal microscopy studies have demonstrated significant morphologic alterations in patients with PES. The eyes with PES is associated with reduced numbers of corneal epithelium, keratocytes, and endothelium as well as reduction in subbasal nerve density. The viscosity and elasticity of the cornea were lower in PES.These corneal alterations may have important assessing the impact of surgical interventions of cataract and treatment of glaucoma patients with PES. Further studies are needed to devise a new PES grading scheme for determining the severity of corneal involvement.
