Özet

Giriş: Hastane infeksiyonlarının önlenmesinde en etkili, en basit ve en ucuz tıbbi yöntem el hijyenidir. El hijyeni uyumunu artırmayı hedefleyen stratejiler içerisinde sağlık çalışanlarının eğitimi önemlidir. Bu çalışmada, bir araştırma hastanesinde alkol bazlı el antiseptiklerine ulaşımın kolaylaştırılıp, personele verilen el hijyeni eğitimi sayısının artırılmasının, uyuma etkisi araştırıldı. Materyal ve Metod: Bu çalışma Ocak 2010-Eylül 2011 tarihleri arasında yapıldı. Gözlemler rutin hasta bakımı sırasında yapıldı. 2010 yılında planlanan dört eğitimden sonra sağlık çalışanlarının el hijyenine uyumu gözlemlendi. Uyumu artırmak için 2011 yılında eğitim sayısı artırıldı ve personelin katılımını artırıcı önlemler alındı. Ayrıca, hastanede bulunan alkol bazlı el antiseptiğine ulaşım kolaylaştırıldı. Müdahale sonrası personelin el hijyeni uyumu üç gözlem döneminde gözlemlenerek müdahale öncesi dönemle karşılaştırıldı. Bulgular: Müdahale sonrası el hijyeni uyumu tüm personel beraber değerlendirildiğinde %29.3'ten %37.2'ye yükseldi (p= 0.0002). Hemşirelerde el hijyeni uyumunun %32.2'den %45'e (p= 0.00007), yardımcı sağlık personelinde %24.7'den %34.3'e yükseldiği görülürken (p= 0.02); doktorlarda istatistiksel olarak anlamlı fark olmadığı (p= 0.81) tespit edildi. Sonuç: Sağlık çalışanlarının el hijyeni uyumu oldukça düşüktür. Eğitimlerin ardından hemşire ve yardımcı sağlık personelinin uyum oranlarında belirgin düzelme saptanırken eğitimlere katılmayan doktorların uyumunda fark saptanmamıştır. El hijyeni uyumunun artırılabilmesi için el hijyeni eğitiminin sürekliliği sağlanmalıdır. Personelin de bu eğitimlere katılımı teşvik edilmelidir.

Abstract

Introduction: Hand hygiene is the most effective, simplest and cheapest method of preventing hospital infections. Training of health care workers is an important step among strategies aimed at increasing hand hygiene compliance. In this study, the effects of facilitating access to alcohol- based hand hygiene antiseptics and of increasing the number of hand hygiene educational programs on compliance were investigated. Materials and Methods: This study was conducted between January 2010 and September 2011. Observations were made during routine patient care. Hand hygiene compliance was observed after four routine hand hygiene educational programs that were planned in 2010. In order to improve compliance, the number of hand hygiene educational programs was increased, and measures to improve staff participation were taken in 2011. In addition, access to alcohol-based hand hygiene antiseptics in the hospital was facilitated. Hand hygiene compliance was observed in three observation periods after the intervention and compared with results before the intervention. Results: Overall hand hygiene compliance increased from 29.3% to 37.2% after the intervention (p= 0.0002). While hand hygiene compliance was increased from 32.2% to 45% among nurses (p= 0.00007) and from 24.7% to 34.3% among the other health care workers (p= 0.02), no statistical difference was observed among physicians (p= 0.81). Conclusion: Hand hygiene compliance among health care workers was extremely low. While significant compliance improvement was observed in nurses and the other health care workers after the educational intervention, no difference was detected among physicians, who did not participate in the training. In order to increase hand hygiene compliance, continuity of hand hygiene training should be ensured, and staff participation in these trainings should be encouraged.
