Tüberküloz lenfadenit olgularından izole edilen mikobakterilerin identifiye edilmesi ve antitüberküloz duyarlılıkları

Özet
Tüberküloz lenfadenit, ekstrapulmoner tüberkülozun en sık görülen klinik formudur ve en çok servikal bölge lenf bezlerini tutmaktadır. Gelişmiş ülkelerde özellikle çocuklarda Mycobacterium tuberculosis dışı mikobakteriler etken olarak daha fazla saptanırken, göçmenlerde ve gelişmekte olan ülkelerde M.tuberculosis en sık saptanan etkendir. Bu çalışmamızda tüberküloz lenfadenit şüphesi ile gönderilen biyopsi örneklerinden mikobakteri izolasyonuna bakteriyolojik yaklaşım, tanı ve tedavinin yönlendirilmesinde mikrobiyoloji laboratuvarının katkısının irdelenmesi amaçlanmıştır. Tüberküloz lenfadenit şüpheli 22 biyopsi örneğinin homojenizasyon sonrası kültürü (Löwenstein-Jensen ve/veya BACTEC 12B) ve yayma preparatları yapılmıştır. Kültürde üreme gösteren örneklerin M.tuberculosis kompleks (MTC) ve nontuberculous mycobacteria (NTM) ayrımları p-nitro-a-asetilamino-b-hidroksi-propiofenon testi ve PCR RFLP yöntemi ile yapılmıştır. MTC üremesi belirlenen izolatların primer antitüberküloz ilaçlara duyarlılıkları belirlenmiştir. Yaşları 13-83 arasında dört kadın olguda ikisi MTC, ikisi NTM olmak üzere mikobakteri izolasyonu yapılmıştır. MTC suşları primer antitüberküloz ilaçlara duyarlı bulunmuştur. NTM izole edilen olgulardan birine cerrahi tedavi, diğer olguya ise ilaç tedavisi uygulanmıştır. Tüberküloz lenfadenit olgularında MTC yanı sıra NTM'in mutlaka düşünülmesi gerektiği, kültür ve identifikasyonunun yapılmasının tedaviyi yönlendirmede katkısının önemli olduğu belirlenmiştir.
Abstract
Tuberculosis lymphadenitis is the most commonly seen form of the extrapulmoner tuberculosis and usually invades the lymph nodules of cervical region. In the developed countries, especially in the children, mycobacteria other than Mycobacterium tuberculosis are much more etiological agents; on the other hand M.tuberculosis is common agent in the immigrant and developing countries. In this study we aimed to establish the contribution of microbiology laboratory to the bacteriologic approach, to the isolation of mycobacteria from tuberculosis lymphadenitis suspected biopsy materials and to the management of the treatment. After the homogenization of 22 biopsy materials with suspected lymphadenitis, culture (Löwenstein-Jensen and/or BACTEC 12B) and smears were made. The differentiation of M.tuberculosis complex (MTC) strains was made with p-nitro-a-acetylamino-b-hydroxy-propiophenone test and PCR RFLP assay. The susceptibility of MTC isolates to primary anti-tuberculosis drugs was determined. From four women patients, age range 13-83, two MTC and two nontuberculosis strains (NTM) were isolated. MTC strains were sensitive to primary anti-tuberculosis drugs. Surgical treatment was performed in one of NTM cases and drug treatment in other one. We reached to the conclusion that in the tuberculosis lymphadenitis cases NTM strains should be absolutely considered together with MTC strains and the culture and identification will be helpful in the management of treatment.
