AKUT HİPOTİROİDİNİN İNTRANAZAL SCHIRMER TEST SONUÇLARINA ETKİSİ

Özet

Amaç: Bu çalışmanın amacı hipotiroidinin nazal nemlilik üzerine etkisini objektif olarak ortaya koymaktır. Yöntem ve Gereçler: Çalışmaya dahil edilen 150 hasta kontrol ve hipotiroidi olmak üzere 2 gruba ayrıldı. Kontrol grubunu total tiroidektomi ve timpanoplasti operasyonu planında olan ötiroid hastalar oluştururken, hipotiroidi grubunu ise tiroidektomi operasyonu sonrası iyatrojenik hipotiroidi gelişen hastalar oluşturmaktaydı. Hastaların demografik bilgileri (yaş, cinsiyet) ve serum sT3, sT4, TSH değerleri kaydedildi. Nazal schirmer testi kontrol grubunu oluşturan hastaların tümüne operasyon öncesi ve hipotiroidi grubuna ise tiroidektomi sonrası hipotiroidi geliştikten sonra bir kez daha yapıldı. Bulgular: Çalışmaya yaş ortalamaları 40,4±11,6 yıl olan 80(%53,3) kadın ve 70(%46,7) erkek toplam 150 hasta dahil edildi. 150 hastanın içinden 41 hastaya tiroidektomi yapıldı ve hipotiroidi grubuna dahil edildi. Hipotiroidi grubu yaş ortalamaları 43,7±11,7 olan 33(%64,7) kadın ve 18(%35,3) erkek hastadan oluşuyordu. Kontrol grubunun preoperatif schirmer test ortalamaları sırasıyla sağ taraf 19,8±6,9 ve sol taraf 19,5±7 olarak saptandı. Hipotiroidi grubunun postoperatif schirmer test ortalamaları sırasıyla sağ taraf 20,2±6,6 ve sol taraf 19±6,6 olarak saptandı. Kontrol ve hipotiroidi gruplarının sağ ve sol nazal kavite schirmer test değerleri arasında yapılan karşılaştırmada istatiksel olarak anlamlı farklılık saptanmamıştır. Sonuç: Birçok nazal semptoma neden olduğu bildirilen hipotiroidinin nasal sekresyon üzerinde kantitatif bir etkisi olmadığı objektif bir yöntem olan nazal schirmer testi kullanılarak ortaya konulmuştur.

Abstract

Objective: This study aimed to reveal objectively the effect of hypothyroidism on nasal hydration. Methods: A total of 150 patients who were included in the study were grouped as the control and the hypothyroid group. The control group consisted of euthyroid patients with planned thyroidectomy and tympanoplasty operations, whereas the hypothyroid group consisted of patients with hypothyroidism after thyroidectomy operation. Demographic data (age-gender) and serum FT3, FT4 and TSH values of the patiens were recorded. Nasal schirmer test was performed on control group before surgery and hypothyroid group after thyroidectomi. Results: The control group consisted of 80(53.3%) female and 70(46.7%) male patients with a mean age of 40.4±11.6 years. The hypothyroid group consisted of 33(64.7%) female and 18(35.3%) male patients with a mean age of 43.7±11.7 years. The preoperative schirmer test averages of the control group were 19.8±6.9 on the right side and 19.5±7 on the left side. The schirmer test averages of the hypothyroid group were 20,2±6,6 on the right side and 19±6,6 on the left side. There was no statistically significant difference between control and hypothyroidism groups in right and left nasal cavity schirmer test values. Conclusion: Hypothyroidism is reported to cause many nasal symptoms isn't a quantitative effect on nasal secretion.
