Yoğun Bakım Örneklerinden İzole Edilen Metisiline Dirençli Staphylococcus aureus Suşlarında Makrolid- Linkozamid-Streptogramin B Direnç Fenotipleri

Özet
Giriş: Bu çalışmada, yoğun bakım ünitelerinde yatan hastalardan izole edilen metisiline dirençli Staphylococcus aureus (MRSA) suşlarında makrolid-linkozamid-streptogramin B (MLSB) direnç fenotiplerinin saptanması amaçlanmıştır. Materyal ve Metod: 2013-2016 yılları arasında yoğun bakım ünitelerinde yatan hastaların kan (45 adet) ve derin trakeal aspirat (5 adet) örneklerinden izole edilen 50 adet MRSA suşu çalışmaya dahil edilmiştir. MLSB direnç fenotipleri, eritromisin (15 µg Oxoid, UK) ve klindamisin (2 µg Oxoid, UK) diski kullanılarak, D test yöntemi ile belirlenmiştir. Eritromisin ve klindamisinin her ikisine de direnç tespit edilmesi yapısal/konstitütif (cMLSB) tipi direnç, eritromisine dirençli suşlarda klindamisin diskinin eritromisin diskine bakan kenarında bir düzleşme olması (D zonu olarak tanımlanan bölgenin oluşması) indüklenebilir (iMLSB) tipi direnç, eritromisine dirençli, klindamisine duyarlı olmasına rağmen D zon görülmemesi efluks (MSB) tipi direnç, hem eritromisine, hem de klindamisine duyarlı suşlar ise S fenotipi olarak değerlendirilmiştir. Bulgular: Çalışmaya alınan 50 adet MRSA suşunun 42 (%84)'si eritromisine, 24 (%48)'ü ise klindamisine dirençli olarak saptanmıştır. Eritromisine dirençli suşlar içerisinde %57.1 (24 suş) oranında cMLSB tipi direnç, %35.7 (15 suş) oranında iMLSB tipi direnç, %7.1 (3 suş) oranında ise MSB tipi direnç saptanmıştır. Sekiz suş hem eritromisine hem de klindamisine duyarlı (S fenotipi) olarak belirlenmiştir. Sonuç: Stafilokoklarda iMLSB direnci mikrobiyoloji laboratuvarında, ucuz fakat güvenilir bir yöntem olan D test ile eritromisin ve klindamisin diskleri kullanılarak kolaylıkla saptanabilir. Eritromisine dirençli stafilokok suşlarında rutin olarak D test uygulanarak, iMLSB direncinin saptanması, klindamisinin uygunsuz kullanımını engelleyecek dolayısıyla tedavi başarısızlıklarının önüne geçilmesinde büyük bir katkı sağlayacaktır.
Abstract
Introduction: In this study, we aimed to evaluate the macrolide-lincosamide-streptogramin B resistance phenotypes of methicillin-resistant Staphylococcus aureus strains isolated from patients in intensive care units (ICUs). Materials and Methods: A total of 50 MRSA strains isolated from clinical samples (45 blood, 5 tracheal aspirate) of patients in ICUs between 2013-2016 were included into the study. Methicillin resistance of the isolates was determined with the use of cefoxitin (30 ?g) disk by Kirby-Bauer disk diffusion method. MLSB resistant phenotypes were investigated by D test method using erythromycin (15 ?g Oxoid, UK) and clindamycin (2 ?g Oxoid, UK) disks. Detecting resistance for both erythromycin and clindamycin were defined as constitutive MLSB (cMLSB), a flattening of the inhibition zone around the clindamycin disk proximal to the erythromycin disk (producing a zone of inhibition shaped like the letter D) as inducible MLSB (iMLSB), non-existence of D zone despite erythromycin resistance and clindamycin susceptibility as MSB, erythromycin and clindamycin susceptible strains as S phenotype. Results: Of all the 50 MRSA strains tested, resistance rates for erythromycin and clindamycin were as 84% and 48%, respectively. Among all tested strains, cMLSB, iMLSB and MSB resistance rates were detected as 57.1%, 35.7% and 7.1%, respectively. Eight strains were susceptible for both erythromycin and clindamycin and defined as S phenotype. Conclusion: iMLSB resistance in staphylococci can be detected easily in the microbiology laboratory by D test, which is an inexpensive but reliable method, by using clindamycin and erythromycin disks. Performing D test routinely in erythromycin resistant staphylococci strains and determining iMLSB resistance will prevent the inappropriate usage of clindamycin and will therefore play an important role in the prevention of treatment failures.
