Prevalence of rotavirus in children with acute gastroenteritis, seasonal distribution, and laboratory findings in the southeast of Turkey

Özet
Amaç: Akut gastroenterit, küçük yaştaki çocuklarda yüksek morbidite ve mortalite ile seyreden önemli bisağlık sorunudur. Rotavirüs grup A, çocukluk çağında en yaygın akut gastroenterit etkenidir. Klinik bulgulavirusa özgü olmadığı için dışkı örneklerinde viral antien analizi yapılması tanı için önemlidir. Bu çalışmada 0-16 yaş grubundaki çocuklarda rotavirüs gastroenteriti sıklığı, yaş ve mevsimsel dağılımı ve laboratuvabulgularının incelenmesi amaçlandı. Gereç ve Yöntemler: Retrospektif olarak düzenlenen çalışmaya, 01 Mart 2012-28 Şubat 2013 tarihleri arasında, akut gastroenterit tanısı alan 3607 hastadan rotavirüs antijeni pozitif saptanan 597 hasta alındKronik hastalığı, immün yetersizliği, malnütrisyonu veya kronik gastroenteriti olan hastalar çalışmaya alınmadı. Rotavirüs gastroenteriti tanısı dışkıda rotavirüs antijen tayini ile konuldu. Dışkı örneklerinde rotavirüs antijen varlığı, kalitatif immunokromotografik yöntem (Simple/Stick Rota Adeno Operon, İspanyakullanılarak, kitin prosedürüne uygun olarak araştırıldı. Testin rotavirus antijenini saptama duyarlılığ%92,2 ve özgüllüğü ise %96,5tür. Olguların laboratuvar verileri hasta dosyalarından kaydedildi. VerileSPSS (Statistical Package for Social Sciences) programı 15,0 kullanılarak analiz edildi. İstatistiksel anlamılık için p değeri <0,05 alındı. Bulgular: Çalışmaya alınan 597 hastanın 356 i (%59,6erkekti. Rotavirüs antijen pozitiflik oranlarının yaş gruparına göre dağılımına bakıldığında ise en yüksek pozitiflik oranının 0-24 ay yaş grubunda %18,8, en düşük pozitiflik oranının ise 12 yaş üstündeki çocuklarda %6,6 olduğu görüldü. Hastaların mevsimsel dağılımlarına bakıldığında; ishal nedeniyle hastaneye başvurula rın en fazla sonbahar ve yaz aylarında, rotavirüs antijen pozitiflik oranlarının ise en sık sonbahar ve ilkbahar aylarında olduğu görüldü. Hemogram incelemelerinde lökositoz (%20), trombosi- topeni (%19,5) ve lökopeni (%12,6) en sık patolojik bulgular idi. Rutin biyokimyasal incelemelerde hiponatremi (%13,4), hipokloremi (%9,6) ve hipopotasemi (%7,8) en sık patolojik bulgular idi. Ayrıca, hastaların 56 (%14,2)sında metabolik asidoz olduğu görüldü. Sonuç: Süt çocuğu gastroenteritlerinde etken olarak rotavirüs sık görülmektedir ve özellikle sonbahar ve ilkbahar mevsimlerinde rutin olarak araştırılmalıdır. Abstract
Objective: Acute gastroenteritis is a major health problem associated with high morbidity and mortalitn children. Group A rotavirus is the most common cause of acute gastroenteritis in childhood. The clinical signs are nonspecific; so, the analysis of viral antigen in the stool specimen is important for the diagnosis. In this study, we aimed to determine the frequencyseasonal distribution, and laboratory findings of rotavius gastroenteritis in children 0-16 years old. Material and Methods: A total of 3607 children who applied between March 2012 and February 2013with complaints of acute gastroenteritis, were invesigated retrospectively. Patients with chronic diseasemmune deficiency, malnutrition, and chronic gastroenteritis were excluded from the study. Rotaviruantigens were detected in the stool of 597 (16.5%patients by qualitative immunochromatographic assay. It has a sensitivity of 92.2% and a specificitof 96.5% for detection of Rotavirus antigenLaboratory findings were recorded from the patientscharts. The frequency of rotavirus gastroenteritis waevaluated according to age and seasonal distributionStatistical analysis was performed using StatisticaPackage of Social Science (SPSS), version 15.0. Results: Viral antigens were determined in 597 16.5%) of 3607 patients. Three hundred fifty-si59.6%) children were male. The incidence of rotavius-positive cases was higher during autumn and spring and higher (18.8%) in 024-month-old pediatic groups (p<0.05). The most common pathologiaboratory findings were leukocytosis (20%), thrombocytopenia (19.5%), and leukopenia (12.6%) in thblood count and hyponatremia (13.4%), hypochlore mia (9.6%), and hypokalemia (7.8%) in the biochemical testAdditionally, 56 (14.2%) of the patients had metabolic acidosis. Conclusion: The results of this study indicated that rotavirus was a frequent cause of acute gastroenteritis in infants, and ishould be routinely, especially during autumn and spring. It is necessary to closely monitor fluid and electrolyte deficiency in patients with rotavirus infections.
