Burden of rotavirus gastroenteritis in the pediatric emergency service

Özet
Amaç: Rotavirüs bütün dünyada çocukluk çağı gast- roenteritlerinin viral etkenleri arasında önemli bir yere sahiptir. Akut gastroenteritler çocuk acil başvuruları- nın önemli bir kısmını oluşturmaktadır. Çalışmamızın amacı; rotavirus gastroenteritlerinin çocuk acil servi- sine olan yükünü araştırmaktır. Gereç ve Yöntemler: Çalışmada 1 Ağustos 2011- 31 Temmuz 2012 tarihleri arasında Ankara Üniversitesi Tıp Fakültesi Çocuk Acil bölümüne başvurup akut gastroenterit tanısıyla, gaitada rotavirüs antijen testi yapılan ve pozitif saptanan hastaların verileri geriye dönük olarak değerlendirilmiştir. Dışkı örnekleri İmmunokromatografik Kart Test (CerTest Rota Kart BİOTEC Zaragoza, İspanya) kiti kullanılarak incelen- miştir. Bulgular: Çalışma süresi içerisinde 3046 gaita örne- ğinde rotavirüs antijeni çalışılmış, 552 olguda pozitif saptanmıştır. Rota virüs saptanan olguların 284ünün erkek, 268inin kız olduğu görülmüştür. Ortanca yaş 1,7 yıl (36 gün- 14,6 yıl) olarak bulunmuştur. Olguların %58,1'ini iki yaş altı olgular oluşturmuştur. Başvurular en sık kış aylarında, en az ise yaz aylarında yapılmış- tır. Hastaların %46'sının acil gözlem odasında veya çocuk enfeksiyon hastalıkları servisine yatırılarak izlendiği ve bu hastaların da %66,8'inin iki yaş altında olduğu, beş yaş üstü hiçbir olgunun hastaneye yatırıl- madığı belirlenmiştir. Sonuç: Rotavirüsün gastroenteritlerinin iki yaş altında ve kış aylarında sıklığının arttığı, yine aynı yaş grubun- da hastaneye yatış sıklığının yüksek olduğu görül- müştür. Yatırılarak tedavi edilen olguların önemli bir kısmı çocuk acil servisi gözlem odasında izlenmiş ve taburcu edilmiş, böylece hastane yataklı servislerinin yükü azaltılmıştır. Çocuk acil servislerinin rotavirus gastroenteritlerinin tedavisinde önemli bir yere sahip olduğu düşünülmüştür.
Abstract
Objective: Rotavirus has an important role in viral childhood gastroenteritis all over the world. Acute gastroenteritis constitutes an important part of pedi- atric emergency department admissions. We aimed to evaluate the burden of rotavirus gastroenteritis in the pediatric emergency department. Material and Methods: Patients who were admitted to Ankara University Faculty of Medicine Pediatric Emergency Department between August 1, 2011 and July 31, 2012 with a diagnosis of acute gastroenteritis were included in this study. Clinical characteristics and the data of patients with a positive rotavirus anti- gen test in stool were analyzed retrospectively. Stool samples were examined by using an immunochro- matographic test (CerTest Rota Kart BIOTEC, Zaragoza, Spain). Results: In the study period, 3046 stool samples for rotavirus antigen were evaluated, and 552 patients (284 boys, 268 girls) were found to have positive test results. The ages of the patients ranged from 36 days to 14.59 years, with a median of 1.70 years; 58.1% of the patients were under 2 years. The patient admis- sions were more frequent in winter and less frequent summer months. It was determined that 46% of the patients were followed up in the pediatric emergency department or pediatric infection disease services; 66.8% of these patients were under 2 years, and no patients were hospitalized over the age of 5. Conclusion: As a result, rotavirus gastroenteritis had a higher frequency under 2 years of age and in winter; also, hospitalizations were more frequent in this age group. Most of the patients were followed up in the pediatric emergency department and discharged after the treatment; thus, the burden of hospital inpa- tient services was reduced. Pediatric emergency departments have an important role in the treatment of rotavirus gastroenteritis.
