Klinik Staphylococcus aureus İzolatlarının Bazı Antibiyotiklere Dirençlerinin Fenotipik ve Genotipik Olarak Belirlenmesi

Özet
Giriş: Staphylococcus aureus, ciddi hastalıklara yol açabilen tehlikeli bir patojendir. Antibiyotik direncinin ortaya çıkması büyük oranda etkili tedaviler bulma sorununa neden olmuştur. Bu çalışma, hastaların çeşitli klinik örneklerinden elde edilen S. aureus izolatlarında bazı antibiyotiklerin antibiyotik duyarlılık paternleri ile antibiyotik direnç genleri arasındaki ilişkiyi değerlendirmek amacıyla yapılmıştır. Materyal ve Metod: Toplam 100 S. aureus klinik izolatı, antimikrobik duyarlılık testine tabi tutulmuştur. Oksasilin (mecA), trimetoprim-sülfametoksazol (TMP-SMZ) (dfrA), eritromisin, klindamisin (ermA, ermB, ermC ve msrA), kinupristin-dalfopristin (vatA, vatB, vatC) ve siprofloksasin (gyrA, gyrB) direnci ile ilişkili genler konvansiyonel polimeraz zincir reaksiyonu (PCR) yöntemi ile amplifiye edilmiştir.Bulgular: Yüz S. aureus izolatı arasındaki metisiline direnç oranı %19 idi ve bu izolatların hepsi de mecA genini taşıyordu. Fenotipik olarak izolatların %1'i TMP-SMZ'ye dirençli iken, yine aynı izolat dfrA genini de taşıyordu. Fenotipik olarak eritromisin direnci %26 olarak tespit edilirken, izolatların %6'sının hem ermA, hem de ermC genini birlikte taşıdığı, %22'sinin ise sadece ermC genini taşımakta olduğu tespit edilmiştir. İzolatların hiçbiri de ermB ve msrA genlerini taşımıyordu. İzolatların hiçbiri fenotipik olarak kinupristin-dalfopristine dirençli değildi ve vatA, vatB, vatC direnç genlerini taşımıyordu. Fenotipik olarak izolatların %6'sı siprofloksasine dirençli iken, genotipik olarak 100 izolatın hepsi de gyrA ve gyrB direnç genlerini taşıyordu.Sonuç: Elde ettiğimiz bulgulara göre, fenotipik antibiyotik duyarlılık testi sonuçları moleküler sonuçlara kısmen benzerlik göstermektedir. Antibiyotik duyarlılığını belirlemede hızlı ve güvenilir yöntemler uygun tedavi kararlarını belirlemek için önemlidir. Bu yüzden klasik yöntemler ve PCR gibi moleküler yaklaşımlar birlikte kullanıldığında daha doğru ve güvenilir bilgiler sağlayabilir
Abstract
Introduction: Staphylococcus aureus is a dangerous pathogen causing serious illnesses. The acquisitions of antibiotic resistance genes has largely led to the problem of finding effective treatments. This study was carried out to evaluate the relationship between the antibiotic susceptibility patterns of some antibiotics and antibiotic resistance genes in S. aureus isolates obtained from various clinical samples of patients. Materials and Methods: A total of 100 clinical S. aureus isolates were subjected to the antimicrobial susceptibility test. The genes associated with resistance to oxacilline (mecA), trimethoprim-sulfamethoxazole (TMP-SMX) (dfrA), erythromycin, clindamycin (ermA, ermB, ermC and msrA), quinupristin-dalfopristin (vatA, vatB, vatC) and ciprofloxacin (gyrA, gyrB) were investigated by PCR amplification. Results: Methicillin resistance ratio of one hundred S. aureus isolates was found 19%, and all of these isolates were carrying the mecA gene. Only 1% of the isolates were phenotypically resistant to TMP-SMX and carrying the dfrA gene. Erythromycin resistance ratio was found 26% phenotypically and 6% of the isolates were carrying both the ermA and the ermC genes, and 22% of them were carrying only the ermC gene. None of the isolates carried the ermB and msrA genes. None of the isolates was phenotypically resistant to quinupristin-dalfopristin and carried vatA, vatB, vatC resistance genes. All isolates were carrying the gyrA and gyrB resistance genes, but 6% of the isolates were phenotypically resistant to ciprofloxacin. Conclusion: According to the acquired results, phenotypic antibiotic susceptibility test results were partially similar to molecular observation. In pathogenic organisms, the use of rapid and reliable methods for determining antibiotic susceptibility is important in determining appropriate treatment outcomes. When biochemical methods and molecular approaches were combined, more accurate and reliable results were observed in a short time.
