Özet

Bu çalışmanın amacı, kateter ile ilişkili infeksiyon etkenlerinin ve bunların bazı antibiyotiklere direncinin belirlenmesidir. Aralık 2003-Haziran 2005 arasında hastanemiz klinik mikrobiyoloji laboratuvarına gönderilen 122 kateter örneğinden koyun kanlı agara yapılan ekimler değerlendirilmiştir. İdentifikasyon için konvansiyonel yöntemler kullanılmış, antibiyotik duyarlılık testleri disk diffüzyon yöntemi ile yapılmıştır. Kateter örneklerinin çoğu nefroloji (n:46) ve cerrahi (n:37) kliniklerinden gönderilmiştir. 122 kateter ucu kültürünün 68’inde (% 56) üreme saptanmış, toplam 76 bakteri ve 7 Candida türü üretilmiştir. Kateterlerden en sık Staphylococcus türleri (45, % 37) izole edilmiş, bunları sıklık sırasına göre Pseudomonas (12, % 10) ve Acinetobacter (10, % 8) türleri izlemiştir. Metisiline dirençli stafilokoklar, diğer antibiyotik gruplarına, metisiline duyarlı olanlara kıyasla, daha dirençli bulunmuştur. Nonfermentatif Gram negatif bakterilerde aminoglikozit ve kinolon grubu antibiyotiklere yüksek oranda direnç saptanmıştır. 12 Pseudomonas suşunun 5’i karbapenemlere dirençli bulunmuştur. Acinetobacter türlerinde karbapenem direnci Pseudomonas’lara oranla daha düşük olmasına rağmen, aminoglikozit ve kinolon direncinin daha yüksek olduğu görülmüştür.

Abstract

The aim of this study was to determine the pathogens in catheter associated infections and their resistance to some antibiotics. 122 catheter tips were delivered to clinical microbiology laboratory between December 2003 and June 2005 included in the study. All of the samples were inoculated on sheep blood agar. Identification were performed by conventional methods. Antimicrobial susceptibility testings were done by the disk diffusion method. Most of the samples were sent from nefrology (n:46) and surgery (n:37) clinics. 68 (56 %) out of 122 catheter cultures were positive. 76 bacteria and 7 Candida spp. were isolated from these cultures. Staphylococcus species (45, 37 %) were the most frequent isolates and followed by Pseudomonas spp. (12, 10 %) and Acinetobacter spp. (10, 8 %), respectively. Metisillin resistant staphylococci were more resistant to other antibiotic groups than methicillin sensitive staphylococci. The resistance to quinolones and aminoglycosides were high in nonfermentative Gram negative bacteria. Five of 12 Pseudomonas strains were found resistant to carbapenems. Carbapenem resistance in Acinetobacter spp. is lower than the Pseudomonas spp., in adverse aminoglycocide and quinolone resistance were higher in Acinetobacter strains.
