Özet

Amaç: Yaşam kalitesini azaltan ve yaşam kaybı ile sonuçlanabilen kalça kırığı, yaşlı nüfusta yaygın görülen ciddi bir sağlık sorunudur. Prospektif olarak gerçekleştirilen bu araştırmada, kalça kırıklı hastaların kırık öncesi ve sonrası yaşadıkları bilişsel, işlevsel ve fiziksel sorunları belirlemek, hasta bilgilerinin hemşire ve doktor kayıtlarına yansıma durumunu değerlendirmek amaçlandı. Yöntem: Araştırmanın örneklemini, Aralık 2003-Ağustos 2004 tarihleri arasında Ankara’da bir kamu hastanesinin ortopedi kliniklerine yatışı yapılan 60 yaş ve üzeri 160 kalça kırıklı hasta oluşturdu. Araştırma verileri, hasta ve yakınlarından, hasta dosyasından ve kayıtlardan toplandı. Bulgular: Araştırmaya katılan hastaların yaş ortalaması 77 yıl olup, çoğunluğu kadındır. Hastaların %58,1’i trokanterik kırık tanısı ile kliniğe yatırıldı ve %93,1’ine cerrahi tedavi uygulandı. Kırık öncesine göre, kırık sonrası hastanede yatarken yetersiz beslenme, yetersiz sıvı alımı, konstipasyon, uyku ve iletişim sorunları yaşama oranının anlamlı oranda arttığı saptandı. Tüm aktivitelerde tamamen başkalarına ve yatağa bağımlılık oranının kırık öncesinde %2,5 olduğu, kırık sonrası bu oranın %56,1’e yükseldiği ve bu hastaların tamamının bağımlı olarak taburcu edildiği belirlendi. Kırık öncesi hastaların %4,4’ünde basınç yarası olduğu, kırık sonrası bu oranın %46,9’a yükseldiği ve bu hastaların %96’sının basınç yarasıyla taburcu edildiği saptandı. Hastaların çoğunluğunun kırık öncesi ve sonrası yaşadığı sorunların ve taburculuktaki durumunun hemşire ve doktor kayıtlarına yansıtılmadığı belirlendi. Sonuç: Araştırma bulguları, kalça kırığı sonrası hastaların beslenme bozukluğu, yatağa bağımlılık ve basınç yarası gibi sorunlar yaşadığını ve bu sorunlar ile taburcu edildiğini ortaya koydu. Ek olarak, mevcut kayıt sisteminin gerektiği gibi işletilmediği saptandı.

Abstract

Objective: Hip fracture that decreases the quality of life and causes mortality is a common severe health problem in the elderly. In this prospective study it was aimed to determine the cognitive, functional and physical problems experienced by the patients before and after fracture, and to evaluate the reflection of patient information to the records kept by the nurses and physicians. Methods: The sample of the study consisted of 160 patients with hip fractures aged 60 years and over who were hospitalized in orthopedic clinics of a public hospital in Ankara between December 2003 and August 2004. Data were collected from patients and their relatives, patient files, and records. Results: The median age of the patients was 77 years and most of them were women, and 58.1% of the patients were admitted to the clinic with trochanteric fracture and 93.1% of them were treated surgically. Compared to pre-fracture period, it was found that complaints of the patients hospitalized due to fractures including malnutrition, insufficient liquid intake, constipation, sleep and communication problems increased meaningfully. It was identified that the rate of dependency to others and bed in all activities was 2.5% before the fracture, and increased up to 56.1% after the fracture, and that all these cases were discharged as dependent-patients. It was found that 4.4% of patients had pressure ulcer before the fracture, that increased up to 46.9% after the fracture, and that 96% of these patients were discharged with pressure ulcer. It was determined that the problems experienced by most of the patients before and after fracture, and the situation in the discharge did not reflect on the records of kept by the physicians and nurses. Conclusion: The results of study also showed that the patients have problems such as malnutrition, dependency to bed and pressure ulcer after hip fracture, and the patients were discharged with these problems. In addition, it was determined that the existing recording system was not operated properly.
