Bazı Enterobacteriaceae türlerine ertapenemin in-vitro etkinliği

Özet
Yeni bir karbapenem grubu antibiyotik olan ertapenemin Escherichia coli, Klebsiella pneumoniae ve Proteus spp. suşlarına in-vitro etkinliği belirlenmiştir. Bakterilerin tanımlanması klasik yöntemler ve Vitek2 tam otomatik bakteri tanımlama sistemi ile, antibiyotik duyarlılıkları disk difüzyon yöntemi ile, GSBL enzimi de çift disk sinerji yöntemi ile araştırılmıştır. Çalışma 81’i (% 62) E.coli, 39’u (% 30) K.pneumoniae ve 10’u (% 8) Proteus spp. olarak tanımlanan toplam 130 suş ile yapılmıştır. Bakterilerden 98’i (% 75) yatan hastalardan, 32’si (% 25) poliklinik takibinde olan hastalardan izole edilmiştir. Suşlarda karbapenemler dışındaki antibiyotiklere yüksek direnç oranları tespit edilmiştir. Yatan hastalardan izole edilen 60 E.coli suşunun 40’ında (% 67), 33 K.pneumoniae suşunun 19’unda (% 58); poliklinik hastalarından izole edilen 21 E.coli suşunun 9’unda (% 43) ve 6 Klebsiella suşunun 2’sinde (% 33) GSBL pozitifliği saptanmıştır. İzole edilen 10 Proteus spp. suşunda GSBL pozitifliğine rastlanmamıştır. GSBL pozitif K.pneumoniae suşlarından 1’i ve GSBL pozitif E.coli suşlarından 1’i imipeneme dirençli olarak bulunmuştur. İmipenem direnci bulunan suşlar ertapeneme de dirençli olarak saptanmıştır. Ayrıca GSBL pozitif, imipeneme duyarlı 2 E.coli suşu da ertapeneme dirençli bulunmuş, böylece hepsi GSBL pozitif 4 suşta (% 3) ertapenem direncine rastlanmıştır. Ertapenem in-vitro olarak E.coli ve K.pneumoniae suşlarına yüksek oranda etkili bulunmuş, bu bakteriler ile oluşan infeksiyonların tedavisinde iyi bir alternatif olabileceğini düşünülmüştür.
Abstract
The aim of this paper is to determine the in-vitro activity of ertapenem, a new carbapenem, for Escherichia coli, Klebsiella pneumoniae and Proteus spp. isolates. The bacteria were identified by Vitek2 microbial identification system and conventional methods, antimicrobial susceptibility tests were performed by disk diffusion method according to CLSI and isolates were examined for ESBL by double disk synergy test. Of 130 clinical isolates, 81 were E.coli (62 %), 39 were K.pneumoniae (30 %) and 10 were Proteus spp. (8 %). Of these strains, 98 (75 %) were isolated from hospitalized patients and 32 (25 %) from outpatients. High resistance rates were found for all antibiotics except carbapenems. Forty (67 %) of 60 E.coli strains, 19 (58 %) of 33 K.pneumoniae strains isolated from hospitilazed patients and 9 (43 %) of 21 E.coli strains, 2 (33 %) of 6 K.pneumoniae strains isolated from outpatients were detected as ESBL producers. None of 10 Proteus spp. strains produced ESBL. Imipenem resistance was detected in 1 E.coli and 1 K.pneumoniae strains, both of which were ESBL positive. These two imipenem resistant isolates were also ertapenem resistant. In addition, 2 other ESBL positive E.coli isolates were also found resistant to ertapenem; so as a total, 4 ESBL positive isolates (3 %) were resistant to ertapenem. Ertapenem demonstrated in-vitro potent activity against E.coli and K.pneumoniae. Ertapenem may be a good alternative for the therapy of infections due to ESBL producing E.coli and K.pneumoniae starins.
