Özet

Darier hastalığı, nadir görülen otozomal dominant geçişli bir genodermatozdur. Tedavisinde kullanılan retinoidlerin kemikte en sık görülen yan etkisi hiperostozistir. Bu olgu sunumunda, Darier hastalığı nedeniyle 3.5 yıldır ikinci nesil bir retinoid olan asitretin kullanan 53 yaşındaki erkek hastada ilaca bağlı gelişen ağrı, güçsüzlük ve ileri derecede kısıtlılığa ve servikal myelomalaziye yol açan hiperostozisten bahsedilmektedir. Uzun süre retinoid kullanımında kas-iskelet sistemi takibinin ihmal edilmesi durumunda işlevleri ileri derecede kısıtlayan nörolojik komplikasyonlar gelişebilmektedir. Dermatologlar tarafından izlenen ve retinoid kullanan hastalar, kas-iskelet sistemine ait olası yan etkiler açısından yakın takip edilmeli ve gerektiğinde fiziksel tıp ve rehabilitasyon hekimine yönlendirilmelidir.

Abstract

Darier's disease is a rare autosomal dominantly inherited genodermatosis. Retinoids are commonly used in its treatment and the most common side effect of retinoids on bone is hyperostosis. In this case report, a 53-year-old man who was taking acitretin, a second generation retinoid, for 3.5 years due to Darier's disease complaining of pain, weakness and severe stiffness secondary to acitretin induced hyperostosis leading to myelomalacia is presented. If musculoskeletal follow up is neglected in long term retinoid use, neurological complications may develop leading to severe functional limitations. Dermatologists should closely follow up their patients using retinoids for possible musculoskeletal side effects and consult to physical medicine and rehabilitation physicians when required.
