Toplum kökenli üriner sistem infeksiyonu etkeni Escherichia coli suşlarında fosfomisin trometamol etkinliği

Özet
Escherichia coli komplike olmayan alt üriner sistem infeksiyonlarından (ÜSİ) en sık izole edilen etkendir. E.coli suşlarında antibiyotik direnç oranlarının artışı, bu patojenin etken olduğu ÜSİ’nun tedavisinde kullanılabilecek alternatif ilaç arayışını daha önemli hale getirmiştir. Bu çalışmada, kadın hastalardan toplum kökenli 169 üriner E.coli izolatının fosfomisin trometamol yanısıra, amoksisilin-klavulanik asit, trimetoprim-sülfametoksazol, siprofloksasin, gentamisin, sefazolin, seftriakson ve imipenem gibi antimikrobiyallere karşı in-vitro duyarlılığı araştırılmıştır. Bakteri tanımlanması konvansiyonel metodlarla yapılırken antimikrobiyal duyarlılık testleri Kirby Bauer disk difüzyon metoduyla ve CLSI kriterleri doğrultusunda belirlenmiştir. İmipeneme direnç saptanmazken sadece bir suş (% 0.6) fosfomisin trometamole dirençli bulunmuştur. Diğer antibiyotiklere direnç oranları trimetoprim-sülfametoksazol için % 58, sefazolin için % 35.5, siprofloksasin için % 34.9 olarak bulunmuştur. Bulgularımızın da gösterdiği gibi fosfomisin trometamol komplike olmayan alt ÜSİ’nin tedavisinde kullanılabilecek güvenilir, etkili ve ucuz bir antibiyotiktir.
Abstract
Escherichia coli is the most commonly isolated microorganism in uncomplicated lower urinary tract infections (UTI). Due to the increased antibiotic-resistance rates in E.coli strains, it is important to have alternative drugs for the therapy of UTIs caused by this pathogen. In this study, we tested the in vitro susceptibility of 169 clinical urinary isolates of E.coli isolated from community acquired women cases to fosfomycin trometamol in comparison with amoxicillin-clavulanate, cefotaxime, cefazolin, ciprofloxacin, gentamicin, imipenem, and trimethoprim-sulphametoxazole. The identification was performed by conventional methods. Antimicrobial susceptibility tests were performed by Kirby Bauer disk diffusion method according to the CLSI criteria. Only one strain (0.6 %) was found resistant to fosfomycin tromethamol, while no resistance was determined against imipenem. The percentages of resistant strains were found to be 58 % for trimethoprim-sulphametoxazole, 35.5 % for cefazolin, and 34.9 % for ciprofloxacin. In conclusion, fosfomycin trometamol seems to be a good alternative that should be considered for the treatment of community acquired lower urinary tract infections.
