Güneydoğu Anadolu Bölgesi’nde Bir Çocuk Hastanesinde Akut Gastroenteritli Çocuk Hastalarda Rotavirüs ve Enterik Adenovirüs Sıklığının Değerlendirilmesi

Özet
Giriş: Günümüzde, tüm dünyada virüsler en yaygın akut gastroenterit etkenleridir. Akut gastroenteritler, özellikle çocuklarda mortalite ve morbiditenin önemli nedenlerindendir. Hem etkili tedavilerinin olmaması hem de gereksiz antibiyotik kullanımı nedeniyle, dışkıda viral gastroenterit etkenlerinin saptanması, hastalığın epidemiyolojisi ve izlenmesi açısından önemlidir. Bu çalışmanın amacı, hastanemize başvuran akut gastroenteritli hastalarda rotavirüs ve adenovirüs sıklığını belirlemektir. Materyal ve Metod: Mayıs 2011-Mayıs 2012 tarihleri arasında Gaziantep Çocuk Hastanesine ishal şikayetiyle başvuran 0-16 yaş arasındaki 8551’i erkek, 6717’si kız çocuktan elde edilen 15.268 dışkı örneği incelenmiştir. Örneklerde rotavirüs ve enterik adenovirüs antijenlerinin varlığı kalitatif immünokromatografik test ile araştırılmıştır. Bulgular: Toplam 15.268 dışkı örneğinin 1926’sında viral antijenler saptanmıştır. Pozitif örneklerin 1741 (%11.4)’i rotavirüs, 185 (%1.2)’i adenovirüs yönünden pozitif bulunmuştur. Rotavirüs ve adenovirüs antijen pozitifliğine en sık 0-2 yaş grubunda rastlanmış olup, oran sırasıyla %58.6 ve %53 olarak saptanmıştır. Ek olarak rotavirüs daha çok kış aylarında, adenovirüs ise bütün yıl boyunca tespit edilmiştir. Sonuç: Akut gastroenterit tanısı alan 0-2 yaş grubundaki her hastada rotavirüs ve adenovirüs antijenlerinin rutin olarak çalışılması büyük önem taşımaktadır. Akut gastroenteritli hastalarda viral etkenlerin saptanması, hem erken tanı ve semptomatik tedavi olanağı sağlayacak hem de ampirik antibiyotik kullanılmasını önleyecektir.
Abstract
Introduction: Today, viruses are the most common agents of acute gastroenteritis all over the world. Acute gastroenteritis, especially in children, is an important cause of mortality and morbidity. Both the lack of effective treatments and unnecessary use of antibiotics make the detection of viral agents in stool important in terms of epidemiology and monitoring of the disease. The purpose of this study is to determine the frequency of rotavirus and adenovirus in patients with acute gastroenteritis admitted to our hospital. Materials and Methods: Between May 2011 and 2012, 15.268 fresh stool specimens obtained from 8551 male and 6717 female children aged between 0-16 years with a diagnosis of gastroenteritis were studied. Rotavirus and enteric adenovirus antigens were investigated by the qualitative immunochromatographic test. Results: Viral antigens were found in 1926 of 15.268 stool samples. 1741 (11.4%) of those positive samples were found to be positive for rotavirus, and 185 (1.2%) of them were found to be positive for adenovirus. Rotavirus and adenovirus antigen positivity was most commonly found in the 0-2 age group and the rate for rotavirus and adenovirus was 58.6% and 53%, respectively. Additionally, rotavirus was more common during winter while adenovirus was observed throughout the year. Conclusion: It is of great importance to search routinely for rotavirus and adenovirus antigen in 0-2 age group with the diagnosis of acute gastroenteritis. Detection of viral agents in patients with acute gastroenteritis will both allow early diagnosis and symptomatic treatment and prevent empirical use of antibiotics.
