Eritroderma ve Ani Sensorinöral İşitme Kaybı Birlikteliği

Özet

Eritroderma, klinik olarak derinin yaygın eritem ve skuamla kaplanmasıdır. Altmışüç yaşında erkek hasta başında, yüz ve gövdesinde kızarıklık, kaşıntı ve kepeklenme şikayetiyle dermatoloji polikliniğine başvurdu. Dermatolojik muayenesinde saçlı deri, yüz ve gövde ön-arka yüzden göbek çevresine kadar yayılan eritemli ve gevşek squamlı plaklar mevcuttu. Lezyonların histopatolojisinde stratum granulozumda bül ve belirgin spongioz, yüzeyel dermiste perivasküler ve interstisyel mikst tipte infiltrasyon mevcuttu. Hastaya oral kortikosteroid tedavisi başlandı. Hastanın yatışının üçüncü gününde sol kulağında ani bir işitme kaybı gelişti. Yapılan odyometride sol kulakta sensorinöral tip işitme kaybı raporlandı. Hastanın takiplerinde eritroderma kliniği ortaya çıktı. Yatışının birinci ayı sonunda deri lezyonları sistemik kortikosteroid tedavisine cevap veren hasta taburcu edildi. Hastamızda eritrodermanın nedenini ortaya koyamadık ancak mevcut bulguları otoimmün bir hastalığı düşündürmekteydi. Deri lezyonları ve işitme kaybı kortikosteroid tedavisine cevap verdi. Burada ani sensorinöral işitme kaybı gelişen bir eritroderma olgusu sunuyoruz.

Abstract

Erythroderma is characterized by generalized erythema and scalling of skin. A 65-year-old male patient was referred to the dermatology clinic with complaints of redness, itching and scalling on his scalp, face and body. Dermatological examination showed erythematous scaly patches involving the scalp, face and trunk. Histopathology revealed cleavage within the granular layer, spongiosis, interstitial and perivascular mixed inflammatory infiltrat in the superficial dermis. The patient was started oral corticosteroid treatment. A sudden loss of hearing occurred in his left ear on the third day of hospitalization. Sensorineural hearing loss has been reported in the left ear by audiometry. Lesions had become eritrodermic. At the end of the first month of hospitalization skin lesions responded to systemic corticosteroid therapy and the patient was discharged. In our patient, we couldn't reveal the cause of the erythroderma but current findings were suggestive of an autoimmune disease. Skin lesions and hearing loss responded to corticosteroid treatment. We present a case of erythroderma by whom sudden hearing loss has occurred.
