Şanlıurfa İlindeki Ter Testi Sonucu Şüpheli Olan ve Klinik Ön Tanısı Kistik Fibrozis Olan Hasta Gruplarındaki CFTR Gen Mutasyonlarının Yeni Nesil Dizileme Yöntemi ile İncelenmesi

Özet
Amaç: Kistik Fibrozis (KF) günümüzde 1/20-25 taşıyıcı frekansı ve 1/2500-3300 görülme oranı ile, beyaz ırktaki otozomal resif geçiş gösteren ölümcül hastalıklar içerisinde ilk sırada yer almaktadır. KF, klor geçirgen transmembran kanal olarak da bilinen kistik fibrozis transmembran regülatör (CFTR) geninde meydana gelen mutasyonlar sonucu açığa çıkmaktadır. Mutasyonların dağılım oranları hasta grupları arasında ve bölgesel olarak farlılık göstermektedir. Bu çalışmada Şanlıurfa ilinde ter testi şüpheli olan ve klinik ön tanısı KF olan bireyler mutasyon dağılımları açısından incelenmiştir. Yöntemler: Ter testi sonucu şüpheli olan 40 hasta ve klinik ön tanısı KF olan 62 hasta yeni nesil sekanslama yöntemi ile CFTR gen mutasyonları açısından değerlendirilmiştir. Bulgular: 40 bireyin yer aldığı ter testi sonucu şüpheli olan hasta grubunda; 11 birey heterozigot (%27,5), 1 birey compound-heterozigot (%2,5) ve 28 birey de homozigot (%70) mutasyona sahip olarak değerlendirildi. 62 bireyin yer aldığı klnik ön tanısı KF olan grupta; 16 birey heterozigot (%25,8), 3 birey compound heterozigot (%4,8) ve 13 birey homozigot (%21) mutasyona sahip olarak değerlendirildi. En sık gözlenen mutasyonlar ise I148T ve ΔF508 olarak bulundu. Sonuç: Çalışmamız hem ter testinin tanısal değerinin klinik değerlendirmeden daha önemli olduğunu hem de ter testi sonucu şüpheli olan bireylerin zaman kaybetmeden araştırılması gerektiğini göstermektedir.
Abstract
Objective: Cystic fibrosis (CF) is the most common autosomal recessive mortal disease in the Caucasians. Its prevalence is 1/2500-3300, and its carrier frequency is 1/20-25. A mutation at the cystic fibrosis transmembrane conductance regulator (CFTR) gene, which is also known as chlorine permeable transmembrane channel, results in CF. There is a difference in the distribution of patients between patient groups and regions. This paper aimed to investigate the mutation dispersion in the state of Sanliurfa in patients who have borderline sweat test results or a clinic suspicion of CF. Methods: A total of 40 patients with borderline sweat test results (first group) and 62 patients with clinically suspected CF (second group) were investigated by next-generation sequencing for CFTR gene mutations. Results: In the first group, 11 heterozygous (27.5%), 1 compound heterozygous (2.5%), and 28 homozygous (70%) mutations were determined. In the second group, 16 heterozygous (25.8%), 3 compound heterozygous (4.8%), and 13 homozygous (21%) mutations were determined. The most frequent mutations were I148T and ΔF508. Conclusion: Our research concludes that the sweat test is more important than clinical suspicion, and every patient with borderline sweat test should be immediately investigated.
