Özet

Predesmetik korneal distrofi (PDKD), derin stroma ve desme membranını tutan, gri-beyaz değişken büyüklükte noktalar ve düzensiz, çizgi şeklinde opasiteler ile kendini gösteren nadir görülen bir kornea distrofi sidir. Sağ gözünde bir haftadır devam eden batma şikayeti ile kliniğimize başvuran 48 yaşındaki erkek hastanın yapılan muayenesinde; her iki gözün düzeltilmiş görme keskinliği Snellen eşeli ile 9/10 seviyesindeydi. Biyomikroskopik muayenede her iki gözde, limbustan limbusa kesintisiz devam eden, arada hemen hiç saydam kornea bırakmayan stromanın arka 1/3 seviyesinden başlayıp desme membranına kadar inen noktasal depozitler izlendi. Fundus muayenesinde her iki optik disk başında eğiklik ve peripapiller koryoretinal atrofi görüldü. Göz içi basınç (GİB) ölçümü sağda 14, solda 15 mm Hg düzeyindeydi. Yapılan pakimetri, korneal topografi , ön segment optik koherans tomografi (OCT) ve konfokal biyomikroskopi sonucunda hastaya predesmetik korneal distrofi tanısı kondu.Literatürde yayınlanmış diğer vakalar tarandığında, dikkat çekici şekilde sık eşlik eden miyopi'nin, bu nadir görülen distrofi nin tanısında yardımcı olabileceği düşünülmelidir.

Abstract

Pre-descemet corneal dystrophy (PDCD) is a rare disorder of the cornea characterized by gray-white punctiform lesions of variable size and irregular linear opacifi cations seen in the posterior stroma.We reported a case of a 48-year-old male who visited our clinic with a complaint of a stinging sensation on the right eye for a week. On routine examination, the best-corrected visual acuity for both eyes was 9/10 with Snellen chart. On slit biomicroscopy, bilateral diffusely distributed punctuate deposits from limbus to limbus with almost no intervening clear spaces, which were located between posterior 1/3 of the stroma and the descemet membrane were observed. On fundoscopy, bilateral optic nerve heads were tilted and peripapillary chorioretinal atrophy was present. The intraocular pressure (IOP) was 14 mm Hg on the right eye, and 15 mm Hg on the left eye. The patient was diagnosed to have pre-descemet corneal dystrophy after ultrasonic pachymetry, corneal topography, anterior segment optical coherence tomography (OCT) and confocal biomicroscopy examinations were completed. We claim that the presence of myopia can be useful in the recognition of this rare dystrophy, since a remarkable coexistence is realized when the past case reports on the literature are considered.
