Hastane kökenli Staphylococcus Aureus türlerinde makrolid-linkozamid-streptogramin B direncinin araştırılmasi

Özet
Staphylococcus aureus suşlarında gelişen antimikrobiyal direnç dünyanın her yerinde nozokomiyal stafilokok infeksiyonlarının tedavisi için önemli problemlere neden olmaktadır. Bu çalışmanın amacı Atatürk Üniversitesi Yakutiye Araştırma Hastanesinde Temmuz 2007-Haziran 2008 tarihleri arasında yatan hastaların çeşitli klinik örneklerinden soyutlanan S.aureus suşlarındaki makrolid-linkosamid-streptogramin B (MLSB) beta direncinin incelenmesidir. Bakteriler konvansiyonel metotlarla tanımlanmıştır. Direnç fenotipleri çift disk difüzyon yöntemi ile araştırılmıştır. Çalışma kapsamına alınan 107 S.aureus suşunun 57’si (% 53) metisiline dirençli S.aureus (MRSA), 50’si (% 47) metisiline duyarlı S.aureus (MSSA) olarak bulunmuştur. MRSA suşlarının % 21’inde yapısal MLSB direnci (cMLSB), % 30’unda ise indüklenebilir MLSB direnci (iMLSB) saptanırken, MSSA suşlarında bu oranlar sırasıyla % 0 ve % 4 olarak bulunmuştur. Klindamisinin stafilokoklarla oluşan infeksiyonların tedavisinde önemli bir alternatif olduğu durumlarda rutin antibiyotik duyarlılık testleri ile direnç profilinin saptanmasının tedavinin başarısı için gerekli olduğu düşünülmüştür.
Abstract
The antimicrobial resistance to Staphylococcus aureus strains causes important problems in treatment of nosocomial staphylococcal infections in all over the world. The purpose of this study was to determine the macrolide-lincosamide-streptogramine B (MLSB) resistance in S.aureus strains isolated from various clinical specimens of the patients hospitalized in Yakutiye Research Hospital of Atatürk University between July 2007 and June 2008. Bacterial strains were identiŞed by conventional methods. Resistance pheno-types were investigated using the double-disk diffusion test. Out of 107 S.aureus strains 57 (53 %) were resistant to methicillin (MRSA) and 50 (47 %) were susceptible to methicillin (MSSA). The constitutive phenotype was 21 % and inducible phenotype was 30 % in MRSA, whereas the constitutive and inducible phenotypes were 0 % and 4 %, respectively, in MSSA. Investigating the resistance patterns on routine antibiotic susceptibility tests provides therapeutic success in S.aureus infections where clindamycin may be an important therapeutic option.
