Özet

Amaç: Karotis endarterektomi (KEA), ciddi karotis arter darlığının tedavisinde uygulanan başlıca revaskülarizasyon yöntemidir. Çalışmamızda, kliniğimizde KEA operasyonuna alınan olgular retrospektif olarak incelenmiştir.Gereç ve Yöntem: 2012-2014 yılları arasında kliniğimizde opere edilen olgular demografik özellikleri, anestezi ve cerrahi yönetim, monitorizasyon yöntemi ve komplikasyonlar açısından retrospektif olarak incelenmiştir.Bulgular: Toplam 99 olgunun 81'i (%81.8) erkek, 18'i (%18.2) kadındı. Ortalama yaş, 66.66±8.96 olarak bulundu. En sıkkullanılan cerrahi tekniğin 44 hasta (%44.4) ile klasik yöntem olduğu görüldü. Yirmi bir (%21.2) olguda genel anestezi uygulanırken, 76 (%76.8) olguda servikal pleksus bloğu uygulandı. İki (%2) olguda ise servikal pleksus bloğu sonrasında genel anesteziye geçildi. Dokuz (%9.1) olguda şant kullanıldı. En çok görülen ek hastalıklar sırasıyla SVO (%71.7), HT (%62.6) ve KAH (%45.5) olarak tespit edildi. Operasyon sırasında 1 olguda ağrı yakınması olurken, 1 olguda bilinç bulanıklığı gelişti. Yetmiş dört hastada (%74.7) serebral oksimetrenin kullanıldığı görüldü. Postoperatif komplikasyonlar açısından bakıldığında 12 hastada (%12.12) geçici komplikasyonların geliştiği, en sık gelişen geçici komplikasyonların ise GİA (Geçici iskemik atak) ve sağ hemiparezi olduğu görüldü. Dört hastada (%4.04) kalıcı komplikasyonların geliştiği, bunların ise sağ pleji+revizyon safen ven interpozisyonu, sol alt ve üst extremitede pleji ve hematom, sol hemipleji, sol kolda pleji olduğu görüldü. Kalıcı komplikasyonlardan ¾'ü genel anestezi, ¼'i servikal pleksus bloğu uygulanan olgulardı. Genel anestezi alan bir olgu ex oldu. Şant kullanılan hastalarda mevcut olan yandaş hastalıklar incelendiğinde bu hastalıkların en sık SVO (n: 8), hipertansiyon (n: 7), koroner arter hastalığı (n: 5), diyabetes mellitus (n: 4) olduğu görüldü. Şant kullanımına göre morbit faktörlerin dağılımı incelendiğinde gruplar arasında istatistiksel olarak anlamlı fark bulunmadı. Serebral oksimetrinin en sık servikal pleksus bloğu uygulanan olgulara kullanıldığı tespit edildi. Yıllara göre anestezi şekilleri açısından gruplar arasında istatistiksel olarak anlamlı fark olduğu ve servikal pleksus bloğunun giderek daha çok tercih edildiği görüldü. Sonuç: KEA'de uygulanacak anestezi metodu açısından kesin bir görüş birliği bulunmamaktadır. Yıllar içinde monitorizasyon yöntemlerindeki yeniliklerle birlikte rejyonel tekniklerin KEA operasyonlarında önemli bir yer edindiğini görmekteyiz

Abstract

Objective: Carotid endarterectomy (CEA) is the main revascularisation method used to treat severe carotid artery occlusion. In our study, we retrospectively examined patients who underwent carotid endarterectomy in our clinic. Material and Method: Patients who had been operated in our clinic between the years 2012-2014 were examined retrospectively in terms of their demographic features, anesthesia and surgical management monitorisation method and complications. Results: Among 99 patients, 81 of them were male and 18 of them were female. Mean age was 66.66±8.96. The mostly used surgical method was classical method in 44 (44.4%) patients. Twenty-one (21.2%) patients had general anesthesia and 76 patient had cervical plexus blockage. In 2 (2%) patients anesthesia method was changed to general anesthesia after cervical plexus blockage. Shunt was used in 9 (9.1%) patients. The most frequently seen comorbidities were CVA (71.7%), HT (62.6%) and CAD (45.5%). During operation one patient felt pain and one patient was unconscious. Cerebral oximetry was used in 74 (74.7%) patients. Temporary complications developed in 12(12.12%) patients. The most frequently developed transient complications were transient ischemic attack (TIA) and right hemiparesis. Permanent complications zdeveloped in 4 (4.04%) patients and these patients had right plegia+revision saphenous vein interposition, plegia and haematoma in the left lower and upper extremities, left hemiplegia, and plegia in left arm. Permanent complications occurred in three-fourths of the patients who received general anesthesia and one-fourth of the patients who received cervical plexus blockage. A patient who had general anesthesia died. When we examined the patients who had undergone shunt procedure, we observed that the most common comorbidities were CVA (n: 8), HT (n: 7), CAD(n: 5), and DM(n: 4). There was no statistically significant difference between morbidity factors related to shunt use. Serebral oximetry was most commonly used in patients who had cervical plexus blockage. When we investigated the anesthesia methods according to the years, we observed that there was statistically significant difference and cervical plexus blockage was more frequently preferred method. Conclusion: There is not an exact consensus on the anesthetic method to be used for CEA.We can say that regional techniques take an important place in CEA surgery in parallel with innovations in the methods of monitoring in time.
