Üriner sistem infeksiyonlarından soyutlanan toplum kökenli escherichia coli suşlarına fosfomisin trometamolün ve bazı antibiyotiklerin in-vitro etkinliği

Özet
Escherichia coli komplike olmayan alt üriner sistem infeksiyonlarından en sık izole edilen mikroorganizmadır. Bu çalışmada toplum kökenli üriner sistem infeksiyonlarından Ekim 2008-Şubat 2009 arasında izole edilen 139 E.coli suşunun fosfomisin trometamol (fosfomisin) de dahil antibiyotik duyarlılıkları ve genişlemiş spektrumlu beta-laktamaz (GSBL) üretimi araştırılmıştır. Suşlarda GSBL üretiminin saptanması, fosfomisin ve sıkça kullanılan antibiyotiklere duyarlılık CLSI önerileri doğrultusunda Kirby-Bauer disk difüzyon yöntemi ile araştırılmıştır. İncelenen 139 E.coli suşunun 36’sında (% 26) GSBL pozitif bulunmuştur. Fosfomisin GSBL üreten ve üretmeyen suşların tümüne in-vitro olarak etkili bulunmuştur. Fosfomisinin E.coli suşlarına etkinliği ile amikasin ve imipenem etkinliği arasında anlamlı bir fark bulunmamış; trimetoprim-sulfametoksazol (p<0.01), gentamisin, siproşoksasin, ampisilin, seftriakson, sefazolinden (p<0.001) ise anlamlı olarak daha etkin bulunmuş, bu antibiyotiklere GSBL pozitif suşlardaki direnç negatif suşlardakine göre anlamlı olarak daha yüksek saptanmıştır (p<0.01). Sonuç olarak, bölgemizde saptanan yüksek direnç oranları da göz önünde bulundurulduğunda, fosfomisin trometamol toplum kökenli idrar yolu infeksiyonlarının tedavisinde tercih edilebilecek bir antibiyotik olarak görülmektedir.
Abstract
Escherichia coli is the most commonly isolated microorganism in cases of uncomplicated lower urinary tract infection (UTI). In this study antibiotic susceptibilities and extended spectrum beta-lactamase (ESBL) activities of 139 E.coli strains isolated in the period of October 2008-February 2009 from community-acquired UTIs were evaluated. ESBL activities and susceptibilities to fosfomycin trometamol (fosfomisin) and to frequently used antibiotics were determined by Kirby-Bauer disk diffusion method according to CLSI standards. Thirty-six of 139 E.coli strains (26 %) produced ESBL. Fosfomycin was found to be active on all ESBL-producing and non-ESBL-producing strains. There was no significant difference between the efficacy of fosfomycin and that of imipenem and amikacin on E.coli strains; however efficacy of fosfomycin was found to be significantly higher when compared with trimethoprim-sulfamethoxazole (p<0.01), gentamicin, ciproşoxacin, ampicillin, ceftriaxone, cefazolin (p<0.001). The resistance of ESBL positive strains against the above mentioned antibiotics was significantly higher than the resistance of the ESBL negative strains (p<0.01). In conclusion, fosfomycin may be an alternative for treatment of UTIs caused by E.coli in our region where resistance rates for frequently used antibiotics are high.
