Bir araştırma hastanesinde cerrahi profilaksi uygulamalarının gözden geçirilmesi

Özet
Cerrahi antibiyotik profilaksisi (CAP) antibiyotiklerin en sık kullanıldığı alanlardan biridir. Ancak, CAP’ta endikasyonsuz antibiyotik kullanılması sıktır. Bu çalışmada, bir eğitim ve araştırma hastanesinin farklı cerrahi kliniklerinde CAP uygulamalarının gözden geçirilmesi amacıyla 9-27 Şubat 2009 tarihinde cerrahi kliniklerinden rastgele seçilen 80 hastanın dosya bilgileri retrospektif olarak değerlendirilmiştir. Temiz kontamine ve kontamine operasyonlar değerlendirmeye alınmıştır. Cerrahi profilaksi rehberlerine uyumsuzluk hata olarak değerlendirilmiştir. Çalışmaya alınan 80 ameliyattan 2’sinde (% 3) hata tespit edilmemiştir. En sık yapılan hatalar, antibiyotik seçiminde (% 69) ve profilaksi süresinde (% 53) idi. Beş operasyonda (% 6) profilaksiye ameliyattan birkaç gün önce başlanmıştı. CAP uygulamalarının 70’inde (% 88) tek antibiyotik kullanılmışken, 10’unda (% 13) ise birden fazla antibiyotik kullanılmıştı. CAP günümüzde sık hata yapılan alanlardandır. Hastanemizde en sık yapılan hata uygun olmayan antibiyotik seçimi ve gereğinden uzun süre profilaksi uygulamasıdır. CAP uygulamasının iyileştirilmesi için hekimlerin bu konuda eğitilmesi, cerrahların, infeksiyon hastalıkları uzmanlarının, mikrobiyologların ve anestezi uzmanlarının işbirliği içinde olmaları gerekir. Ayrıca her hastanenin kendi direnç paternini dikkate alarak cerrahi profilaksi kılavuzlarını oluşturması, cerrahların da bu rehberlere uyumları sağlanmalıdır.
Abstract
Surgical antibiotic prophylaxis is one of the most frequently used indications for antibiotics. But, sometimes antibiotics may be used inappropriately for this reason. This study is designed to evaluate prophylactic antibiotic use and determine the errors if there are about this issue among different surgical clinics in a newly established education and research hospital. A randomly 80 patients were selected in eight surgery clinics of our hospital at date 9-27 February 2009, retrospectively. Only the clean contaminated and contaminated operations were included. These patients files were evaluated according to current guidelines. An error is evaluated as incompliance to surgical prophylaxis guide. No surgical antibiotic prophylaxis errors were detected in only two (3 %) operations in 80 operations. The most common errors was wrong antibiotic selection for surgical prophylaxis with a rate of 69 % and duration of prophylaxis which was longer than enough with a rate of 53 %. Antibiotic prophylaxis was started a few days before for five (6 %) of the operations. Single antibiotic was started for 70 operations (88 %), where more than one antibiotic were started for 10 operations (13 %). Antibiotic prophylaxis for surgery is an important issue where errors can be made commonly in the use of antibiotics. The most common mistakes in our hospital were inappropriate antibiotic selection and duration of prophylaxis longer than needed. To improve the enforcement of antibiotic prophylaxis for surgery; training of the physicians in this regard and co-operation of surgeons, infectious disease experts, microbiologists, and anesthetists must be provided. In addition, each hospital must create its own guidelines for surgical prophylaxis in consideration with its own resistance patterns and the surgeons must be convinced to trust and follow these guidelines.
