Birinci Basamakta Lyme Artriti; Olgu Sunumu

Özet

Birinci basamak sağlık hizmetleri hastaya en yakın olan noktadır. Hastalanan veya sağlığından şüphe duyan kişinin ilk başvurduğu kişi, Aile hekimi, Aile Sağlığı Merkezi veya kendine en yakın sağlık kurumudur. Bu noktadaki doğru teşhis ve tedavi hasta ve sağlık hizmetlerinin etkinliği ve tedavinin maliyeti açısından büyük önem taşır. Lyme artriti, İxodes cinsi sert kenelerin ısırığı ile geçen Borrelio Burgdorferi spiroketinin neden olduğu hastalıklardan biridir. Enfeksiyonlarında deri, merkezi sinir sistemi, kalp, göz ve diğer organlar tutulabilir, çoğu olguda eklemler başlıca tutulum yeridir. Artrit şikayetleri ile başvuran çocukların çok küçük bir yüzdesi Lyme artriti hastasıdır. Avrupa da ise çocuk ve ergenlerde bakteriyel enfeksiyonu takiben ortaya çıkan artritler içinde en sık görüleni Lyme artritidir. Lyme artritinin başlıca semptomları, etkilenen eklemlerde şişlik, hareket kısıtlılığı ve sıvı birikimidir. Sıklıkla, eklem şişliklerine ağrı eşlik eder. Bu olgu sunumunda, birinci basamağa eklemlerde ağrı, şişlik, yürüyememe şikayeti ile gelen Lyme artritli bir hastayı sunduk.

Abstract

Primary health care is the closest point to the patient. The person who is ill or suspicious of his / her health goes to the family physician, the family health center or the closest health institution. The correct diagnosis and treatment at Primary health care center is of great importance in terms of the cost effectiveness and public health improvement.Lyme arthritis is one of the diseases caused by Borrelio Burgdorferi, spirochete passing through the bite of the hard ticks of Ixodes. Infections may involve the skin, central nervous system, heart, eye and other organs, while in most cases joints are the main site of involvement. A very small percentage of children presenting with arthritis complaints have Lyme arthritis. In Europe, the most common form of arthritis following bacterial infection in children and adolescents is Lyme arthritis. Frequently, joint swelling is accompanied by pain. In this case report, we present a patient with Lyme arthritis who complained of pain, swelling, and inability to walk.
