Özet

Amaç: Primer konjenital glokomlu (PKG) gözlerdeki korneal değişimlerin in vivo konfokal mikroskopi (İVKM) ile değerlendirilmesi amaçlanmıştır. Gereç ve yöntemler: Kocaeli Üniversitesi Tıp Fakültesi Glokom Departmanı’nda PKG tanısıyla takipli 8 hastanın 13 gözü çalışma kapsamına alındı ve 20 kontrol göz ile karşılaştırıldı. En iyi düzeltilmiş görme keskinliği (EİDGK), santral korneal kalınlık (SKK), ön kamara derinliği (ÖKD), horizontal kornea çapı (HKÇ), aksiyel uzunluk (Aks) ve İVKM ile bazal epitelyal hücre, anterior ve posterior stromal keratosit ve endotel hücre dansiteleri ölçül- dü. İVKM yapılabilmesini engelleyecek düzeyde skarlı kornealar çalışma dışı bırakıldı. Bulgular: SKK, PKG grubunda 547.29 ± 51.93μm, kontrol grubunda 562.65 ± 28.55μm, (p=0.301) ve ÖKD, PKG grubunda 4.14 ± 0.74mm, kontrol grubunda 3.43 ± 0.27mm, (p=0.005) olarak ölçüldü. HKÇ, PKG grubunda istatistiksel anlamlı olarak yüksekti (p<0.001). İVKM değerlendirmesinde PKG grubunda bazal epitelyal hücre dansitesi 5582 ± 488 hücre/mm 2 kontrol grubunda 6249±531 hücre/mm 2 (p=0.003), arka keratosit dansitesi PKG grubunda 338 ± 82 hücre/mm 2 kontrol grubunda 486 ± 109 hücre/mm 2 (p=0.042) ve endotelyal hücre dansitesi PKG grubunda 2203 ± 630 hücre/mm 2 kontrol grubunda 3317 ± 551 hücre/mm 2 olarak saptandı ( p<0.001). Ön keratosit hücre dansitesinde gruplar arasında anlamlı bir değişim saptanmadı (p=0.803). Stromada anormal morfolojik yapıda sinir fi brilleri izlendi. Endotelyal hücrelerde pleomorfi zm bazı buftalmik gözlerde dikkat çekiciydi. Sonuç: Korneal tabakalardaki bu dansite azalması ve endotel hücrelerinde gösterilen pleomorfi zm hastaların takibinde ve cerrahi kararı öncesinde akılda tutulmalıdır.

Abstract

Purpose: To describe the clinical features and microstructural characteristics assessed by in vivo confocal microscopy (IVCM) in patients with primary congenital glaucoma (PCG). Material and methods: Thirteen eyes of eight patients with clinically diagnosed PCG were enrolled and compared with twenty control corneas in Kocaeli University Medical Faculty Ophthalmology Clinic Glaucoma Department. Clinical parameters; best corrected visual acuity (BCVA), central corneal thickness (CCT), anterior chamber diameter (ACD), corneal diameter (WTW), axial length (Ax) and IVKM parameters; the cell density of basal epithelium, keratocytes and endothelium were compared with control corneas. The scarred corneas were excluded. Results: CCT was measured as 547.29 ± 51.93μm in the PCG group, 562.65 ± 28.55μm in the control group (p=0.301) and ACD was measured as 4.14 ± 0.74 mm in the PCG group and 3.43 ± 0.27mm in the control group (p = 0.005). There was statisticaly signifi cant difference between groups in terms of WTW (p<0.001). Basal epithelial cell density was 5582±488 cell/mm 2 in PCG group and 6249±531 cell/mm 2 in control group (p=0.003). There were statistically signifi cant difference in terms of posterior keratocytes and endothelial cell densities between PCG (338±82 cell/mm 2 , 2203±630 cell/mm 2 , respectively) and control groups (6249±531 cell/mm 2 , 3317±551 cell/mm 2 , respectively), (p=0.042, p<0.001, respectively). There was no signifi cant change in anterior keratocyte cell density (p=0.803). Stromal nerve fi brils were observed in the abnormal morphological structure. Endothelial cell pleomorphism was notable in some buphthalmic eyes. Conclusion: This diminished density in the corneal layers and pleomorphism in endothelial cells should be kept in mind following the patients and before the surgical decision.
