Özet

Amaç: Pektus ekskavatum (PE), ön göğüs duvarının en sık görülen kongenital deformitesidir. PE onarımında minimal invaziv teknikle pektus ekskavatum yöntemi "Nuss Tekniği" yaygınlaşmış ve son yıllarda tüm dünyada en sık kullanılan yöntemdir. Kliniğimizde PE tanılı olgularda uygulanan minimal invaziv yöntemle PE operasyon tekniği ve postoperatif dönem sonuçları açısından değerlendirildi.Yöntem: 2007 - 2014 yılları arasında kliniğimizde opere edilen PE tanılı 30 olgu retrospektif incelendi.Bulgular: Olguların 26'sı erkek 4'ü kadın olup yaş ortalaması 17.63 (12.9-27.3) idi. Üç olgu daha önce açık yöntemle (Ravitch) operasyon uygulanmış nüks olgular iken kalan 27 olgunun ilk başvurusu idi. Tek bar yerleştirilmesi 28 olguda yeterli oldu, iki olguda çift bar uygulandı. Operatif mortalite yoktu, morbidite %16.6 olup en sık görülen komplikasyon iki olguda drenaj gerektiren pnömotoraks idi. Hastanede ortalama kalış süresi 3.8 gün (2-7 gün) ve postoperatif ortalama izlem süresi 35.6 ay (14-73 ay) izlendi. Yaşam kalitesi anketinde olguların %93.4'ü tedaviden memnun olduğu görüldü.Yorum: PE deformitelerinin düzeltilmesinde minimal invaziv yöntem, kozmetik sonuçları ve yüksek hasta memnuniyeti ile güvenli ve etkili bir yöntemdir

Abstract

Background: Pectus excavatum (PE) is the most common congenital deformity of the anterior thoracic wall. The minimally invasive repair of pectus excavatum (MIRPE), "Nuss technique", is the most frequently performed technique world wide. Our objective was to review minimally invasive repair of pectus excavatum in our clinic. Method: Between 2007 - 2014, 30 patients who were operated in our clinic with the diagnosis of PE, were investigated retrospectively.Results: Twenty six cases were male, 4 were female. Median age was 17.63 (12.9-27.3). Three cases had a previous open repair (Ravitch technic). While 1 bar placement was enough in 28 cases, in 2 cases double bar placed for PE correction. There was no mortality. Morbidity were detected as 16.6%. The most common complication was pneumothorax in 2 cases and drainage was performed in both cases. The length of hospital stay ranged from two to 7 days, with a mean of 3.8 days, respectively. The mean follow-up period was 35.6 months. Quality-of-life questionnaires revealed that 93.4% of patients were satisfied with the treatment.Conclusion: This minimally invasive technique for PE is a promising and effective procedure for better cosmetic results and high levels of patient satisfaction
