Parsiyel Üreter Obstrüksiyonunda Tadalafilin Etkisi

The Effects of Tadalafil on The Partial Ureteral Obstruction

Özet

Amaç: Farklı deneysel modellerde yapılan çalışmalarda, Fosfodiesteraz-5 inhibitörlerinin böbrekler için koruyucu olduğu gösterilmiştir. Bu çalışma-da spesifik bir fosfodiesteraz-5 inhibitörü olan tadalafilin parsiyel üreter obstrüksiyonundaki olası renoprotektif etkisi incelenmiştir. Gereç ve Yöntem: Erkek Sprague-Dawley sıçanlar sham (S), tek taraflı parsiyel obstrüksiyon (P) ve tadalafil tedavisi (10 mg/72 saat, intragastrik) alan obstrüksiyon grubu (PT) olmak üzere, üçe ayrılmıştır (n =6). Obstrüksiyondan 2 hafta sonra, serum ve idrar kreatinin ile üriner sistatin-c ölçümü yapılmış, GFR hesaplanmıştır. Histolojik incelemeyle böbrekteki dejeneratif değişimler değerlendirilmiştir. Sonuçlar, ortalama±standart sapma olarak verilmiştir. İstatistiksel karşılaştırmalarda one-way ANOVA ve Bonferroni testi kullanılmış, p <0,05 olduğunda fark önemli kabul edilmiştir. Bulgular: Serum kreatinin konsantrasyonu, S grubuyla kıyaslandığında (0,30±0,03 mg/dL), P grubunda (0,48±0,10 mg/dl; p <0,01) ve PT grubunda (0,41±0,06 mg/dl; p <0,01) yüksek bulunmuştur. S grubunda GFR değeri 2,56±0,56 ml/dk iken, P grubunda 1,40±0,25 ml/dk (p <0,001) ve PT gru-bunda 1,71±0,21 ml/dk’ya düşmüştür (p <0,01). Üriner sistatin-c/kreatinin oranı S grubunda 0,96±0,23 olarak hesaplanmıştır. Bu oran, S grubu ile kıyaslandığında, P grubunda (2,34±0,50; p <0,001) ve PT grubunda (1,41±0,38; p <0,05) yüksek bulunmuştur. P ve PT grubu arasındaki fark istatis-tiksel olarak önemlidir (p <0,05). Yapılan histolojik incelemeler, P grubunda, tübüler ve glomerüler hasar, intersitisyel fibrozis ve inflamasyon oldu-ğunu göstermektedir. PT grubunda ise glomerüler yüzey alanının P grubundan daha yüksek olduğu (p <0,05), tübüler hasar ve fibrotik değişimlerin ise daha az olduğu görülmüştür. Sonuç: Bu çalışmada, tek taraflı üreter obstrüksiyonu sırasında yapılan tadalafil tedavisinin renal hasarı önemli ölçüde azalttığı ve proksimal tübül fonksiyonlarında iyileşme sağladığı gösterilmiştir. Taşa bağlı üreter obstrüksiyonlarının klinikteki görülme sıklığı düşünüldüğünde, tadalafilin bu etkisinin klinik olarak önem taşıdığı düşünülmektedir.

Abstract

Objective: Previous studies in different experimental models have shown protective effects of phosphodiesterase-5 inhibitors for kidneys. In the present study, the possible renoprotective effect of tadalafil, a specific phosphodiesterase-5 inhibitor, was examined during partial ureter obstruction. Material and Method: Male Sprague-Dawley rats were divided into 3 groups (n =6) as sham (S), unilateral partial obstruction (P) and tadalafil treated (10 mg/72 hours, intragastric) obstruction (PT). Two weeks after obstruction, creatinine measurements performed in serum and urine, urinary cystatin-c levels were measured and GFR values were calculated. Histological examination was performed to evaluate degenerative changes in the kidney. The results are given as mean±standard deviation. Statistical comparisons were performed by one-way ANOVA and Bonferroni test, with p <0.05 accepted as significant difference. Results: The serum creatinine concentration increased in the P group (0.48±0.10 mg/dL; p <0.01) and the PT group (0.41±0.06 mg/dL; p <0.01) when compared to S group (0.30±0.03 mg/dL). The GFR value was 2.56±0.56 ml/min in the S group, and decreased to 1.40±0.25 ml/min (p <0.001) in the P group and 1.71±0.21 ml/min (p <0.01) in the PT group. Urinary cystatin-c/creatinine ratio in the S group was calculated as 0.96±0.23. This ratio increased in the P group (2.34±0.50; p <0.001) and the PT group (1.41±0.38; p <0.05) compared to the S group. The difference between the P and PT groups was statistically significant (p <0.05). Histological examination revealed the tubular and glomerular damage, interstitial fibrosis and inflamma-tion in the P group. In the PT group, the glomerular surface area was higher compared to the P group (p <0.05), with less tubular damage and fibrotic changes. Conclusion: Present study showed that tadalafil treatment significantly reduced renal damage and improved proximal tubule function during unilat-eral ureteral obstruction. Considering the incidence of stone-linked ureter obstruction, it is thought that this effect of tadalafil may have clinical im-portance.
