Gebelerdeki HBsAg Taramasının ve Seropozitifliğinin Yıllar İçindeki Değişimi

Özet
Giriş: Gebelikte HBsAg’nin rutin bakılması gebelik komplikasyonlarının yönetiminde ve yenidoğanın korunmasında hayati öneme sahiptir. Ülkemizde 1998 yılında rutin aşılama programına geçilmesi ve 2014 yılında doğum öncesi bakım rehberinin yayınlanmasıyla tüm gebelere takip sürecinde HBsAg bakılması önerilmektedir. Çalışmamızın amacı Ege bölgesinin göstergesi olabilecek yüksek miktarda gebe bakılan ve doğum yaptırılan ikinci basamak bir hastanede yıllara göre gebelerde HBsAg taranma durumunu ve sonuçlarını değerlendirmektir. Materyal ve Metod: Çalışmaya 1 Temmuz 2009-1 Ocak 2019 tarihleri arasında Buca Kadın Doğum ve Çocuk Hastalıkları Hastanesinde doğum yapmış veya kadın hastalıkları ve doğum polikliniklerinde en az bir kez muayene olmuş tüm gebeler dahil edilmiştir. Mükerrer muayeneler dışlandığında toplam 142.869 gebenin retrospektif olarak ne kadarında hastanemizde HBsAg bakıldığı ve yıllara göre dağılımı, HBsAg pozitifliği olan gebelerin yıllara ve yaş gruplarına göre dağılımı incelenmiştir. Bulgular: Hastane kayıtlarına göre başvuran toplam 142.869 gebenin %16.7’sinde HBsAg bakılmıştır. HBsAg 366 (%1.5) gebede pozitif saptanmıştır. Gebelerde HBsAg bakılma oranı yıllara göre %1-24 arasında değişmektedir. Yaş gruplarına göre değerlendirildiğinde HBsAg pozitifliği %0.5-4.3 arasında değişmektedir. En düşük pozitiflik oranı %0.5 ile 17 yaş altı gebeliklerde, en yüksek pozitiflik oranı %4.3 ile 45 yaş üstü gebeliklerde saptanmıştır. Sonuç: Merkezimizde gebelere HBsAg bakılma oranının en yüksek %24 olduğu görülmesine rağmen hastanemiz protokolü gereği hastane kayıtlarında HBsAg sonucu olmayan ve dış merkezde yapılan HBsAg tetkikini gösteremeyen doğum için müracaat eden her gebeye HBsAg bakılmaktadır; bakılan hasta sayısına göre tarama oranının düşük olarak saptanmasının nedeni hastaların HBsAg tetkikini başka bir merkezde yaptırmış olmasıdır. Gebelerde HBsAg pozitifliğinin saptanması anne ve bebek sağlığı için hayati bir öneme sahiptir, hastaların gözden kaçmaması ve gereksiz tetkikle ülke ekonomisine zararın önlenmesi için sağlık sisteminin tüm basamaklarında HBsAg sonuçlarının birbirine entegrasyonu gerekmektedir.
Abstract
Introduction: Routine examination of HBsAg during pregnancy is vital in the management of pregnancy complications and protection of the newborn. After the hepatitis B vaccination program since 1998 and publication of the prenatal care guidelines in 2014, HBsAg screening is recommended for all pregnant women. The aim of our study is to evaluate HBsAg screening status and results in pregnant women in second stage hospital with high pregnancy rate, which may be indicative of Aegean region of Turkey. Materials and Methods: The study included all pregnant women who had been examined in the outpatient clinics and have given birth at İzmir Buca Maternity and Children Hospital between 1 July 2009 and 1 January 2019. After the exclusion of repetitive examinations, 142.869 pregnant women were evaluated retrospectively according to years and age groups in terms of HBsAg screening and HBsAg positivity. Results: HBsAg screening was performed in 16.7% of 142.869 pregnant women who admitted to the hospital. HBsAg were detected positive in 366 (1.5%) pregnancies. The incidence of HBsAg screening in pregnant women ranged between 1% and 24% according to the years. HBsAg positivity ranged from 0.5% to 4.3% when evaluated according to age groups. The lowest HBsAg positivity rate was found 0.5% in pregnancies under 17 years old, and the highest positivity rate was 4.3% in pregnancies over 45 years old. Although the rate of HBsAg screening up to 24% in pregnant women in our center, If there is no HBsAg result that can be demonstrated by the pregnant woman who is admitted to the hospital for the first time for delivery, the HBsAg test is requested for each pregnant woman in this condition according to the hospital protocol. The reason for the low rate of screening according to the number of pregnant women is because the pregnant women have had the HBsAg screening in another center. Conclusion: In conclusion determining HBsAg positivity in pregnant women is important for maternal and infant health. HBsAg screening results must be integrated in all stages of the health care system in order to prevent patients from being overlooked and to prevent cost to the national economy through unnecessary examinations.
