Stafilokok suşlannda slaym faktör pozitifliği, metisilin ve antibiyotik direnci

Özet

Çeşitli klinik örneklerden Haziran 2009-Mart 2011 arasında izole edilen Staphylococcus aureus ve koagülaz negatif stafilokoklarda (KNS) metisilin direnci, “slime” (slaym) oluşumu ve antibiyotik direnci araştırılmıştır. Çalışmaya 139 S.aureus, 94 KNS suşu alınmıştır. Toplam 233 suşun 182’si (% 78.1) yara ve apse, 43’ü (% 18.5) kan, altısı (% 2.6) trakeal aspirat örneğinden, ikisi (% 0.9) göz sürüntüsünden izole edilmiştir. Konvansiyonel yöntemlerle izolasyon ve identifikasyonu yapılan suşların antibiyotik duyarlılıkları disk difüzyon yöntemiyle; slaym yapımı ise Kongo kırmızılı agar yöntemi kullanılarak araştırılmıştır. S.aureus suşlarının 106’sı (% 76) metisiline duyarlı, 33’ü (% 24) metisiline dirençli; KNS suşlarının 64’ü (% 68) metisiline duyarlı, 30’u (% 32) metisiline dirençli bulunmuştur. S.aureus suşlarında slaym pozitifliği % 39, KNS suşlarında % 30 oranında saptanmıştır. Hiçbir suşta glikopeptid direnci saptanmamıştır. Slaym oluşturan stafilokoklar arasında antibiyotik direnci daha yüksek bulunmamış, aksine slaym oluşturmayanlarda metisilin ve antibiyotik direnç oranları daha yüksek saptanmıştır. Sonuç olarak, hastanemizde izole edilen stafilokok suşlarındaki slaym yapımı, S.aureus ve KNS suşlarında benzer bulunmuştur. Ancak bu çalışmada, slaym oluşturmayan suşlarda metisilin direnci ve paralel olarak antibiyotik direnç oranları daha yüksek saptanmıştır.

Abstract

Methicillin and antibiotic resistance, and slime production of Staphylococcus aureus and coagulase negative staphylococci (CNS) isolated from various clinical specimens were investigated. During the period of June 2009 and March 2011, 139 S.aureus and 94 CNS strains isolated from various clinical specimens were included in the present study. Out of the 233 isolates 182 (78.1 %) strains were obtained from abscess and wound samples, 43 (18.5 %) from blood, six (2.6 %) from tracheal aspiration and two (0.9 %) from eye swap samples. Isolation and identification procedures were performed by conventional methods. Antibiotic susceptibilities were searched by disk diffusion method while slime production was searched by Congo red agar method. Among 139 S.aureus isolates, 106 (76 %) were found susceptible to methicillin and 33 resistant (24 %); 64 (68 %) CNS strains were found susceptible to methicillin and 30 resistant (32 %). Slime production was detected in 39 % of S.aureus and in 30 % of CNS strains. All isolates were susceptible to glycopeptides. Antibiotic and methicillin resistance were found higher among slime nonproducers in contrast to the expectation. These results indicated that, slime production ratio among S.aureus and CNS were similar in our hospital. However, methicillin and antimicrobial resistance were significantly higher in slime negative strains in this study.
