Melatonin Premedikasyonunun Epidural ID Anestezi Yapılan Hastalarda İntraoperatif ve Postoperatif Hemodinamik Parametreler, Periferik Oksijen Satürasyonu, Postoperatif Anksiyete ve Kognitif Fonksiyonlar Üzerine Etkilerinin İncelenmesi

Özet
Amaç: Anksiyete preoperatif dönemde çoğu hastanın yaşadığı semptomlar topluluğudur. Premedikasyon bu semptomlar topluluğunu azaltır. Bu çalışmada epidural kateter takılan ve elektif TUR-prostat ameliyatı olacak hastalarda preoperatif melatoninin intraoperatif vital bulgulara, postoperatif anksiyete ve kognitif fonksiyonlara etkisini incelemeyi amaçladık. Yöntem: Bu çalışma, 60 yaş üstü 80 erkek olguda gerçekleştirildi. Hastalar rastgele 40’ar kişilik iki gruba ayrıldı. Grup 1 (n=40) ameliyattan 60 dk. önce melatonin 6 mg verildi. Grup 2 (n=40) premedikasyon yapılmadı. TUR-Prostat hastalarına epidural kateter yerleştirildikten sonra, epidural kateterden 3 mL % 1’lik lidokain test dozunu takiben, kateterin yeri doğrulandıktan sonra preoperatif 20 dk. önce epidural kateterden 12-18 mL % 0.5’lik levobupivakain yapıldı. Bulgular: Gruplar karşılaştırıldığında postoperatif SAB, DAB, OAB ve KAH değerleri arasında anlamlı fark saptanmadı. Gruplar periferik oksijen saturasyonu açısından karşılaştırıldığında, Grup 1’in SpO2 değerlerinin, Grup 2 değerlerinden anlamlı derecede yüksek olduğu tespit edildi. Gruplar MMT açısından karşılaştırıldığında Grup 1’in postoperatif 1. saat MMT değeri, Grup 2’nin postop 1. saat MMT değerinden anlamlı derece yüksek bulundu. Sonuç: Epidural blok öncesi hastalara melatonin verilmesinin kognitif fonksiyonları ve hemodinamik parametreleri olumlu etkilediği ancak anksiyete üzerine olumlu ya da olumsuz etkisinin bulunmadığı kanaatine varıldı.
Abstract
Objective: Anxiety is a collection of symptoms experienced by most patients in the preoperative period. Premedication reduces the collection of these symptoms. In this study, we aimed to investigate the effects of preoperative melatonin administration on intraoperative vital signs, postoperative anxiety and cognitive functions in patients with epidural catheter who will undergo elective under epidural anesthesia. Methods: This study was performed on 80 male patients older than 60 years. The patients were randomized into 2 groups as follows: Group 1 (n=40) patients received 6 mg melatonin 60 minutes before the surgery, while Group 2 did not receive premedication. After placement of the epidural catheter to all TUR-P patients, following test dose of 3 mL 1% lidocaine administered after placement of epidural catheter, and confirmation of its location, 12-18 mL 0.5% levobupivacain was delivered 20 minutes before operation. Results: A significant intergroup difference was not found as for postoperative HR, SAP, DAP, MAP values when the. SpO2 values. In Group 1 peripheral oxygen saturation was significantly higher than Group 2 values The postoperative 1st hour MMT value of Group 1 was significantly higher than the postoperative 1st hour MMT value of Group 2. Conclusion: We have concluded that use of melatonin in patients before epidural block effects cognitive functions and hemodinamic parameters positively, without any positive or negative effect on anxiety.
