Polimiksin B' nin imipeneme dirençli Acinetobacter baumannii suşlarına karşı in-vitro aktivitesi

Özet

Acinetobacter baumannii’ye en etkili antibiyotikler içerisinde yer alan karbapenemlere karşı direncin son yıllarda giderek arttığı görülmüştür. İmipeneme dirençli suşlar genellikle sadece polimiksinlere duyarlı kalıp çoklu ilaç direnci gösterirler. Tedavi seçeneklerinin sınırlı olması nedeniyle antibiyotik duyarlılık testlerinde A. baumannii’nin en duyarlı bulunduğu grup olarak karşımıza çıkan polimiksinlerin [polimiksin B ve polimiksin E (kolistin)] kullanımı gündeme gelmiştir. Bu çalışmada Ocak-Aralık 2007 tarihleri arasında çeşitli klinik örneklerden soyutlanan imipeneme dirençli 78 A.baumannii izolatına karşı polimiksin B’nin etkinliğinin E-test yöntemi ile in-vitro olarak belirlenmesi amaçlanmıştır. Değerlendirilen 78 A.baumannii izolatının polimiksin B için MİK değer aralığı 0.064-128 μg/ml (MİK50: 0.25 μg/ml, MİK90: 0.75 μg/ml) olarak tespit edilmiş, CLSI standartlarına göre 78 suşun 76’sı (% 97) duyarlı, 2’si (% 3) dirençli MİK değerleri göstermiştir. Sonuç olarak çalışmamızda polimiksin B için tespit etmiş olduğumuz % 3’lük direnç oranı dünya literatüründeki verilerle uyumlu bulunmuş, hastanemizde izole edilen çoklu ilaç direnci gösteren A.baumannii’lerde polimiksin B için direnç problemimiz olmadığı görülmüştür. Yüksek duyarlılık oranları bu antibiyotiği önemli bir seçenek haline getirse de çoklu dirençli A.baumannii infeksiyonlarında halen etkinliği ve güvenilirliği kanıtlanmış herhangi bir antibiyotik bulunmamaktadır. Bu nedenle bu antibiyotiğin klinik kullanımdaki yerini, kombinasyon tedavilerindeki etkinliğini ve yan etkilerini netleştirmeye yönelik klinik araştırmalara ihtiyaç vardır.

Abstract

Continuously evolving resistance to carbapenems which are considered as the most effective antimicrobials against Acinetobacter baumannii, is being detected recently. These imipenem resistant strains usually exhibit multi-drug resistance remaining consistently susceptible only to polymyxins. Because of limited therapeutic options, clinicians have returned to the use of polymyxins [polymyxin B (PB) or polymyxin E (colistin)] which are being detected as the most active agents based on the results of in vitro susceptibility testing of the A. baumannii isolates. The aim of this study was to determine the in vitro activity of polymyxin B by E-test method, against 78 imipenem resistant A.baumannii strains isolated from clinical specimens between January and December 2007. MIC values of tested strains for polymyxin B were between 0.064-128 μg/ml (MIC50: 0.25 μg/ml, MIC90: 0.75 μg/ml). Resistance to polymyxin B was detected in only 2 (3 %) strains, whereas the remaining 76 (97 %) were fully susceptible to polymyxin B according to CLSI criteria. In conclusion, the resistance rate of 3 % is in accordance with the published reports in the literature and this data suggests that resistance to polymyxin B is currently relatively rare among multi drug resistant A.baumannii isolates in our hospital. Although high sensitivity rates put forward this antibiotic as an important choice, there is no therapy that has been proven to be effective and reliable for patients with multidrug resistant A.baumannii infections on clinical grounds since there is limited clinical data. Thus, the need for a systematic clinical evaluation is evident to clarify the clinical utility, effectiveness in combination therapies and possible side effects of this antibiotic.
