Özet

Amaç: Cerrahi alan enfeksiyonları (CAE), alınan birçok önleme rağmen hala modern cerrahinin çok önemli ve ciddi bir problemi olmaya devam etmektedir. Ciddi mortalite, morbidite artışına neden olmakta, hastanede kalış süresini uzatmakta ve çok önemli miktarda ekonomik kayıplara neden olmaktadır. Uygulanan cerrahi yöntem ile CAE insidansı arasında yakın bir ilişki vardır. Acil müdahale gerektiren kolorektal operasyonların çoğu kontamine girişim olduğundan, bunlarda CAE riskleri daha yüksektir.Materyal ve Metod: Çalışmamızda 2011- 2014 yılları arasında acil olarak opere ettiğimiz toplam 66 kolorektal patolojisi bulunan hastanın dosyasını retrospektif olarak inceledik. Cerrahi alan enfeksiyonu gelişen hastalara yapılan işlemleri temiz/ kontamine veya kontamine olmalarına göre sınıflandırdık.Bulgular: Dört yılda acilden opere edilen kolorektal cerrahi hasta sayısı 66 ve yaş ortalaması 65,79±12,17 idi. Temiz/ kontamine sınıfında 40, kontamine sınıfında 26 hasta mevcuttu. Bu hastalardan 30'unda postoperatif morbidite ve bunlarında 24'ünde CAE geliştiği görüldü. Cerrahi alan enfeksiyonu gelişen hastalardan 18'i kontamine sınıfındaydı.Sonuç: Cerrahi alan enfeksiyonu gelişen hastaların büyük çoğunluğu kontamine ve obezite idi. Ayrıca regüle olmayan diabetik ve kronik akciğer hastalarında riskin belirgin arttığı gözlendi

Abstract

Aim: Surgical site infections (SSI) despitevariousprecautionsstillremainto be a veryimportantandserious problem of modern surgery. Itcauses a severe increase in both mortality and morbidity extends the length of hospital stay and leads to significant economic losses. There is a closerelationbetween SSI incidenceandthetype of thesurgicalprocedure. Due to most of the emergency colorectal operations are contaminated interventions, they have higher risk of SSI. Material and method: In our study, the files of 66 patients with colorectal disease who underwent emergency surgery between 2011 and 2014 were studied retrospectively. The procedures that had been applied to the patients with SSI were classified whether clean/contaminated or contaminated.Results: The number of the patients who under went emergency colorectal surgery in 4 years was 66 and the mean age was65,79±12,17. Therewere 40 patients in clean/contaminated class and 26 patients in contaminated class. 30 of these patients had postoperative morbidity and 24 of these had SSI. 18 of the cases with SSI were classified as contaminated.Conclusion: The vast majority of the cases with SSI were contaminated and obese. It was also observed that the risk is significantly increased in patients with uncontrolled diabetes and chronic lung disease
