Hipertansif Hastalarda Kardiyovasküler Risk Faktörü olarak Ürik Asit ile CRP, RDW, MPV Arasındaki İlişkinin Değerlendirilmesi

Özet

Amaç: Esansiyel hipertansiyonda hem hastalığın etyopatogenezi hem de hastalıkla ilişkili klinik sonuçlar üzerinde etkili birçok faktör bildirilmiştir. Bu faktörlerden birisi de ürik asittir. Bu çalışmada bu hasta popülasyonunda kardiyovasküler riskin erken öngörülebilmesinde serum ve spot idrar ürik asit düzeyleri ile kan kırmızı hücre dağılım genişliği (RDW), ortalama trombosit hacmi (MPV), C-reaktif protein (CRP) arasında ilişki araştırılması amaçlanmıştır.Yöntemler: 1 Ocak 2013-31 Aralık 2013 tarihleri arasında esansiyel hipertansiyon tanısı ile takipli 62 hasta çalışma kapsamına alındı. Hastalarda kardiyovasküler hastalık varlığı ekokardiyografi, koroner anjiografi ve efor testi ile değerlendirildi. Kardiyovasküler komplikasyon gelişmeyenler (grup 1), gelişenler (grup 2) olarak belirlendi. Serum ve spot idrarda ürik asit, serumda CRP, MPV ve RDW değerleri için uygun örnek alınarak iki grup karşılaştırıldı.Bulgular: Çalışmamıza katılan hastaların 23'ünde (%37,1) kardiyovasküler komplikasyon saptandı. Grup 1 ve grup 2 CRP düzeyi 4,89 ve 3,64; RDW: 13,8 ve 13,5; serum ürik asit düzeyi 5,57±1,23, spot idrarda 65,13±29,11 ve serumda 5,33±0,8, spot idrarda 61,94±18,13; MPV seviyeleri 7,7 ve 7,2 olarak ölçüldü. İki grup arasında serum ve spot idrar ürik asit, serum CRP, MPV ve RDW değerleri açısında anlamlı olarak farklılık saptanmadı (p>0,05). Yapılan çapraz karşılaştırmalarda grup 2'de RDW ile glomeruler filtrasyon hızı ve RDW ile MPV arasında negatif yönde ve CRP ile RDW arasında pozitif yönde anlamlı bir ilişki saptanmıştır (sırasıyla; p<0,01, p<0,05, p<0,05).Sonuç: Esansiyel HT'lu hastalarda serum ve spot idrar ürik asit, serum CRP, MPV ve RDW düzeyleri hipertansif hastalarda kardiyovasküler hastalık gelişimini öngörmede etkili değildir.

Abstract

Objective: Many factors that are associated with both clinical outcomes and pathogenesis of disease in cases of essential hypertension have been reported. One of these is uric acid. In this study, we aimed to investigate the relationship between serum and spot urine uric acid levels and red blood cell distribution width (RDW), mean platelet volume (MPV), and C-reactive protein (CRP) levels in patients with essential hypertension.Methods: Sixty-two patients with essential hypertension were enrolled in the study between January 01, 2013 and December 31, 2013. The presence of cardiovascular disease was assessed by echocardiography, coronary angiography, and stress test. The patients were divided into two groups according to the absence (group 1) or presence (group 2) of cardiovascular complications. Data were collected for serum and spot urine uric acid levels, serum CRP levels, MPV, and RDW and were compared between the two groups.Results: Cardiovascular complications were determined in 23 patients (37.1%). In groups 1 and 2, the CRP level was 4.89 and 3.64, RDW was 13.8 and 13.5, serum uric acid level was 5.57±1.23 and 5.33±0.8, spot urine uric acid level was 65.13±29.11 and 61.94±18.13, and MPV was 7.7 and 7.2, respectively. A significant difference was not found between the two groups in terms of serum and spot urine uric acid levels, serum CRP levels, MPV, and RDW (p>0.05). In group 2, there was a significant negative correlation between the glomerular filtration rate and RDW and between RDW and MPV and positive correlation between CRP levels and RDW (p<0.01, p <0.05, and p<0.05, respectively).Conclusions: Serum and spot urine uric acid levels, serum CRP levels, MPV, and RDW cannot predict the development of cardiovascular disease in patients with essential hypertension.
