Özet

Bu çalışmada; 1995-2009 yıllarında, menenjit şüpheli hastaların beyin omurilik sıvısı (BOS) örneklerinden izole edilen bakteriler ve antibiyotik duyarlılıklarının araştırılması amaçlanmıştır. Üreme saptanan 262 BOS örneğinden en sık izole edilen etken Streptococcus pneumoniae (% 23) olmuştur. Bunu sırasıyla koagülaz negatif stafilokok (% 21), Acinetobacter spp. (% 10), Neisseria meningitidis (% 9), Enterobacteriaceae üyeleri (% 9), Pseudomonas spp. (% 7), Staphylococcus aureus (% 5), Haemophilus influenzae (% 3), Candida spp. (% 2) ve Enterococcus spp. (% 2) izlemiştir. 1997 ve 2000 yılları arasında pik yaptığı gözlenen S.pneumoniae suşlarında, penisilin için MİK aralığı 0.008 - 0.38 µg/ml iken, dört suş penisiline dirençli bulunmuş, beş suş azalmış duyarlı olarak (E-test ile MİK 0.064 - 0.094 µg/ml) bildirilmiştir. 2006 yılına dek en sık Eylül ayında izole edilen N.meningitidis suşları bu tarihten sonra izole edilmemiştir. Ciddi komplikasyonlara yol açabilen bakteriyel menenjitte izole edilen etkenler ve antibiyotik duyarlılıklarının yıllar içindeki değişimlerinin bilinmesi uygun tedavinin düzenlenmesinde yol gösterici olacaktır.

Abstract

The goal of this study is to investigate the antibiotic resistance of the bacteria isolated from cerebrospinal fluid (CSF) samples of the patients with suspected meningitis between 1995 and 2009. In 262 CSF samples with positive cultures, the most frequently isolated agent is Streptococcus pneumoniae by 23 %. It is followed by coagulase negative staphylococci (21 %), Acinetobacter spp. (10 %), Neisseria meningitidis (9 %), Enterobacteriaceae strains (9 %), Pseudomonas spp. (7 %), Staphylococcus aureus (5 %), Haemophilus influenzae (3 %), Candida spp. (2 %), and Enterococcus spp. (2 %). MIC range for penicillin in S.pneumoniae strains with a peak between 1997 and 2000 is 0.008 to 0.38 μg/ml and 4 strains are found to be resistant, 5 strains are reported as decreased susceptibile (MIC 0.064 - 0.094 μg/ml by E-test). N.meningitidis strains were most frequently isolated in September until 2006 and they were not observed after that date. Bacterial meningitis can cause series complications. Understanding the isolated agents and their antibiotic resistance according years will help to determine the proper treatment.
