290 Olguda Mide Karsinomlarının Klinikopatolojik Analizi

Özet

Amaç: Etyolojisin de birçok çevresel ve genetik etkenler bulunan mide karsinomları, biyolojik, genetik ve morfolojik olarak heterojen özellikler gösteren malign epitelyal neoplazmlardır. Dünya genelinde tüm kanserlerin %7,8'ini oluşturan gastrik kanserler, teşhis ve tedavideki çok sayıda yeni yaklaşıma rağmen, halen başlıca ölüm nedenleri arasındadır. Bu çalışmada, 10 yıl içerisinde mide karsinomu nedeniyle gastrektomi uygulanan 290 olgunun retrospektif olarak klinikopatolojik açıdan incelenmesi amaçlandı.Gereç ve Yöntemler: 2004-2014 yılları arasında Şişli Hamidiye Etfal Eğitim ve Araştırma Hastanesi, Patoloji Kliniği'nde tanı almış mide karsinomlarının klinik ve histopatolojik değerlendirilmesi yapıldı. Bulgular: Bu çalışma serisindeki 290 hastanın 190'ı erkek (%65), 100'ü kadın (%35) olup, en genç hasta 29, en yaşlı hasta 107 yaşındaydı. Operasyon şekli 184 olguda total gastrektomi, 104 olguda subtotal gastrektomi, 2 olguda parsiyel gastrektomiydi. Patolojik evre 150 olguda pT4, 100 olguda pT3, 18 olguda pT2, 22 olguda pT1 olarak saptandı. Olgulardan 21'i iyi diferansiye, 97'si orta derecede diferansiye, 172'si kötü diferansiye idi. Lauren sınıflamasına göre 50 olgu diffüz tip, 207 olgu intestinal tip gastrik karsinom olarak sınıflandırılırken, 33 olguda diffüz ve intestinal tip bir arada görüldü. Olgulardan 225'inde (%78) lenf bezi metastazı mevcuttu.Sonuç: 10 yıllık gastrik karsinomların klinikopatolojik analizi ile olguların pek çoğunun ileri evrede saptandığı görülmüş olup erken evrede tanı koyarak daha iyi sonuçlar elde edilmesine yönelik yeni çalışmalara ihtiyaç olduğunun altı çizilmektedir.

Abstract

Objective: Gastric carcinomas are heterogeneous malignant epithelial neoplasms with numerous biological, environmental and genetic etiological factors. Gastric cancers are 7.8% of all cancers worldwide. Even though new aspects in diagnosis and treatment, gastric cancers take one the highest death toll. In the current study, 290 gastrectomized gastric cancers were investigated with the clinicopathological aspects retrospectively in a 10-year-period.Material and Method: A detailed clinical and histopathological investigation has been done with the gastric carcinomas which were diagnosed in the pathology department of Istanbul Sisli Hamidiye Etfal Training and Research Hospital between the years 2004 and 2014.Results: In this series, out of 290 cases 190 were male (65 %), and 100 were female (35 %). The youngest was 29 years old and the eldest was 107 years old. 184 of the cases underwent total gastrectomy, 104 underwent subtotal gastrectomy, 2 underwent partial gastrectomy. In 150 cases the pathological stage was pT4, in 100 cases the stage was pT3, in 18 cases the stage was pT2, in 22 cases the stage was pT1. 21 of the cases were well-differentiated, 97 were moderately differentiated, 172 were poorly differentiated. According to the Lauren classification, 50 cases were diffuse type, 207 were intestinal type, 33 were mixed type. There was lymph node metastasis in 225 cases Conclusion: In the 10-year clinicopathological analysis of the gastric carcinoma, most of the cases were diagnosed at an advanced stage. It is underlined that further studies are needed for an early diagnosis and better results.
