Alternatif tedavi seçeneği olarak fosfomisinin komplike üriner sistem infeksiyonlarından izole edilen GSBL pozitif escherichia coli suşlarına etkinliği

Özet
Escherichia coli, üriner sistem infeksiyonlarının en sık rastlanan patojenidir. Hastane kökenli suşlardan sonra toplumdan izole edilen üriner sistem etkeni E.coli izolatlarında da saptanan siprofloksasin direnci ve genişlemiş spektrumlu beta-laktamaz (GSBL) üretimi, yeni tedavi seçeneklerine olan ihtiyacı ortaya koymaktadır. Bu prospektif çalışmada; hastanemiz Mikrobiyoloji laboratuvarına 2010 yılının ilk dokuz ayında gönderilen idrar örneklerinden, komplike üriner sistem infeksiyonu etkeni olarak izole edilen 416 E.coli izolatının GSBL üretimi ile siprofloksasin ve fosfomisin duyarlılığı disk difüzyon yöntemi ile araştırılmıştır. Tüm izolatların % 25’inde GSBL üretimi, yarısında siprofloksasin direnci saptanırken, % 97’si fosfomisine duyarlı bulunmuştur. GSBL üreten izolatlarda fosfomisin duyarlılığı, GSBL negatiflere benzer bulunmuştur (% 95 ve % 98). Üriner infeksiyonlarda en sık izole edilen patojen E.coli olup siprofloksasin de bu hastalarda en sık reçete edilen antibiyotiktir. Bu yüzden E.coli suşlarında artan siprofloksasin direnç oranları global bir problemdir. Eski bir antibiyotik olan fosfomisin, son yıllarda toplum kökenli üriner sistem infeksiyonlarının tedavisinde önerilmektedir. Bu ajanın, özellikle ayaktan takip edilen, siprofloksasine dirençli E.coli suşları ile infekte ve komplike üriner sistem infeksiyonlu hastalar için alternatif bir tedavi seçeneği olabileceği düşünülmüştür.
Abstract
Escherichia coli is the most common agent of urinary tract infections (UTI). Emerging extended spectrum beta-lactamase (ESBL) production and ciprofloxacin resistance patterns in community acquired E.coli isolates after the nosocomial ones has resulted in searching for new and effective alternative therapy choices for UTI. In this prospective study, ESBL production, ciprofloxacin and fosfomycin susceptibilities of 416 E.coli strains isolated from complicated UTIs within the first nine months of 2010 were investigated. ESBL production was detected in 25 % and ciprofloxacin resistance in 52 % of all strains while 97 % of them were susceptible to fosfomycin. The fosfomycin susceptibilities of ESBL producing and non-producing strains were similar (95 % and 98 %). Ciprofloxacin is the most common antibiotic used in UTIs caused by E.coli. So, emerging ciprofloxacin resistance among E.coli isolates is a global problem. Fosfomycin -an old antibiotic- is now a new agent that advised for the treatment of community acquired UTI. We thought that, this agent may be an alternative therapy choice for both ciprofloxacin resistant and ESBL producing E.coli infections in out-patients with complicated UTI.
