Atrofik Vaginit Tedavisinde Uzun Dönem Düşük Doz 17-β Estradiol Vaginal Tablet Uygulamalarının; Kli- nik Etkinliğiİ ve Endometrial Sonuçları

Özet
Östrogen yetmezliğine bağlı atrofik vaginiti olan 81 postmenopozal kadın açık kontrollü bir çalışmada 25μg’lık 17β-östradiol ile iki farklı dozda intravaginal olarak tedavi edildi. Hastaların tümü başlangıçta 2 hafta boyunca günlük tedavi aldı. Daha sonra idame tedavisi olarak rastge- le ya haftada bir(39 hasta) ya da haftada iki(42 has- ta) vaginal uygulamaya toplam 1 yıl boyunca devam edildi. Tedavi başlangıcından önce ve tedaviden bir yıl sonra endometrial histopatoloji değerlendirildi. Tedaviden hemen önce ve tedavi izleminde 6. ve 12. aylarda subjektif semptomlar, klinik atrofik vaginit bulguları ve östrogen-gonadotropin düzeyleri üzerin- deki etkiler saptandı. Tedavi öncesi endometrial biopsilerin hepsi atro- fik endometriyumu gösteriyordu. Haftada bir teda- vi edilen gruptaki bir yıllık tedaviyi tamamlayan 39 hastadan 33’ünde (%84.6) atrofik endometriyum gözlendi. Sadece 4(%10.25) hastada hafif proliferatif ve 2(%5.12) hastada proliferatif olmak üzere toplam 6(%15.38) hastada östrogene bağlı hafif endometri- yal etkilenme görüldü. İdame tedavisi olarak hafta- da iki doz rejimi uygulayanlarda ise 35(%83) hastada atrofik endometriyum, 5(%11.9) hastada hafif proli- feratif ve 2(%4.76) hastada da proliferatif endomet- riyum gözlendi. Haftada 2 doz rejimi hemen hemen hastaların tü- münde semptomları tümüyle giderirken haftada bir kez tedavi olan grupta hastaların çoğunun hala orta şiddette semptomları vardı. Her iki tedavi rejimi uzun süreli güvenli tedavi sağlamakla birlikte, postmenopozal östrogen yetmezliği ile ilgili ürogenital semptomların ortadan kalkması gözönüne alındığında haftada iki dozluk rejimin haftada bir dozluk tedaviye oranla üstün olduğu gözlendi.
Abstract
Eighty one postmenopausal women with atrophic vaginitis due to estrogen deficiency were treated int- ravaginally with 25 mikrog 17 B-estradiol using two different regimes in an open controlled study Initially all patients recieved a daily vaginal regime for two weeks long. Then, maintanence therapy was carried out after randomisation to either once a week (39 patients) or twice a week (42 patients) regimes that continued for a year. The endometrial biopsy samples of the cases were assessed histopat- hologically before the initiation of the therapy and after completion of one year of the treatment. The effects of the treatment on the subjective symptoms, the findings of clinical atrophic vaginitis and estro- gen-gonadotrophin levels were recorded before the initiation of the therapy and at 6th and 12th months of the therapy. Evaluation of all endometrial biopsy samples obtai- ned initially revealed atrophic endometrium.Atrop- hic endometrium was observed in 33 of 39 patients (84.6%) who completed one year of once a week regime.A total of only 6 patients (15,38%) had mild endometrial proliferation due to estrogen and four of these patients were considered to have a mild pro- liferation(10.25%). In patients who received twice a week regime, 35(83%) patients had endometrial at- rophy , 5 patients(11.9%) had mild endometrial pro- liferation and 2 patients(4.76%) had moderate endo- metrial proliferation. The symptoms resolved completely in almost all pa- tients who received twice a week regime while most of the patients who received once a week regime had ongoing moderate symptoms. Although both treatment regimes provided long term safety,twice a week regime was observed to be superior to once a week regime in resolvation of uro- genital symptoms associated with postmenopausal estrogen deficiency.
