Özet
Amelogenezis imperfekta (Aİ) süt ve daimi dentisyonu etkileyen otozomal dominant veya otozomal resesif geçiş gösteren kalıtsal bir hastalıktır. Prevelansı araştırılan topluma göre değişmekle beraber 1:700 ile 1:14000 arasında görülmektedir. Hipoplastik, hipomatür, hipokalsifiye ve hipomatür-hipoplastik olmak üzere 4 farklı tipte sınıflandırılır. Aİ hastaları benzer ağız içi komplikasyonlara sahiptirler. Mine defektleriyle beraber dişlerde hassasiyet, dikey boyut kaybı ve estetik şikayetlere sık rastlanır. Hastaların karmaşık estetik ve fonksiyonel gereksinimleri multidisipliner çalışmayı gerektirir. Bu olgu sunumunda 23 yaşında hipomatür Aİ teşhisi konulan bayan hastada başlangıç periodontal tedavi sonrasında üst çenede kuron boyu uzatma işlemini takiben tüm dişlerde tam seramik restorasyonlar yapıldı. Süreç boyunca hastada protetik sebeplerden ötürü endodontik tedavi ihtiyacı olmadı. Tedavilerin bitiminden sonra hasta 4 aylık aralıklarla idame programına alındı ve 12. ay kontrolünde herhangi bir problem saptanmadı. Hasta hala klinik takibimiz altında olup, artan çiğneme fonksiyonu ve restorasyonlarının estetik görüntüsünden oldukça memnundur.
Abstract
Amelogenesis İmperfecta (AI) is a hereditary disorder that affects primary and permanent teeth enamel and can be both autosomal dominant or autosomal recessive. The prevalence varies from 1:700 to 1:14000 according to the populations studied. AI may be subdivided as hypoplastic, hypomature, hypocalcified and hypomature- hypoplastic. Regardless of the type, all AI patients have similar intra-oral complications such as sensitivity due to the enamel defects, loss of vertical dimension and esthetic problems. Complex esthetic and functional requirements of the patients make multidiciplinary approach obligatory. In this case presentation, 23 year-old female patient with hypomature AI was treated with full mouth ceramic restorations after crown lengthening of the upper jaw. No endodontic treatment was necessary for prosthetic purposes. At the end of the treatment, patient was recalled to maintenance program every 4 months and at 12-month follow up, no problem was detected. She is still under our clinical follow-up and pleased with both esthetic and function.
