Mikrodiskektomi Cerrahisinde Uyguladığımız Antibiyotik Profilaksisinin Etkisinin Retrospektif Değerlendirilmesi

Özet

Amaç: Spondilodiskitisi önlemede profilaktik antbiyotik kullanımı çok önemlidir. Bu çalışma; mikrodiskektomi ameliyatı uyguladığımız hastalarda kullandığımız profilaktik antibiyoterapinin etkinliğini değerlendirmek için yapılmıştır. Yapılan bu retrospektif klinik çalışmanın sonuçlarının, literatüre katkı sağlayacağı düşünülmüştür. Gereç ve yöntem: Ocak 2011- Aralık 2015 yılları arasında kliniğimizde mikrodiskektomi uygulanan her hastaya standart olarak; indüksiyon sırasında intravenöz(iv)sefazolin veya siprofloksasin uygunlamış olup intraoperatif disk aralığı rifamisin ile irrige edilmiştir. Tüm hastalara standart mikrocerrahi işlemi aynı cerrahi ekip ile standart mikrodiskektomi presedürü uygulanmıştır. Postoperatif 1-3 gün arasında peroral olarak sefuroksim veya siprofloksasin antibiyoterapi profilaksisi için kullanılmıştır. Bu hastaların birinci hafta, birinci ay, üçüncü ay, altıncı ay ve birinci yıl takipleri yapılmış. Spondilodiskitis kliniği olan hastalara eritrosit sedimentasyon hz (ESR) ve C-reaktif protein değerleri ölçülmüş, anlamlı yükseklik olan hastalara kontrastlı spinal mrg yapılmıştır. Bulgular: Bu seride komplikasyon olarak postoperatif dönemde iki hastada enfeksiyon gelişmiştir. Bir hastada cerrahi insizyon altında cilt altı ve kas dokusunda gelişen yara yeri enfeksiyonu bir hastada da diskitis gelişmiştir. Bu serinin diskitis oranı %0,3’dür. Sonuç: Literatürde benzer bir çalışmada rifamisinli salinli yıkama sıvısı ile cerrahi lojun irrigasyonu yapılmıştır. Bu çalışmanın spondilodiskitis oranı %0,67’dir. Sunduğumuz çalışmanın literatürden farkı ise sulandırılmadan direkt olarak rifamisin ile disk aralığının irrigasyonudur.İyatrojenik spondilodiskitis, tedavisi maliyetli ve daha çok antiyotik kullanımı gerektirmektedir. Bu sebeple intraoperatif uygulanacak rifamisin ve post-operatif üç günlük peroral antibiyoterapinin spondilodiskitisten korunmada ve daha maliyetli tedavilerin önüne geçmede etkili olduğunu düşünüyoruz.

Abstract

Objective: Spondylodiscitis following discectomy is a potentially life-threatening complication. Prophylactic use of antibiotics is crucial in preventing spondylodiscitis. The present study aims to evaluate the effectiveness of prophylactic antibiotherapy in patients undergoing microdiscectomy. It is thought that the results of this retrospective clinical trial will contribute to the literature. Materials and Method: A total of 241 patients who underwent microdiscectomy at our clinic between January 2011 and December 2015 received intravenous (i.v.) cefazolin and ciprofloxacin during induction of anesthesia as a standard of care and rifamycin was administered introperatively into the disc space. Antimicrobial prophylaxis was continued with peroral cefuroxime and ciprofloxacin in the postoperative period between days 1 and 3. All patients were scheduled for clinical follow-up visits at one week, one, three, and six months, and one year after surgery. Erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) values were obtained in patients with the symptoms of spondylodiscitis and the patients with clinically significant findings underwent contrast- enhanced spinal magnetic resonance imaging (MRI). Results: In present mikrodiscectomy series, two patients developed postoperative infection. In one patient, wound infection developed under subcutaneous and muscular tissue under surgical incision and discitis developed in one patient. The rate of spondylodiscitis was 0.3% in the present series of patients. Conclusion: In a similar study in the literature, rifamycin was used with saline irrigation with surgical region. The rate of spondylodiscitis was %o,67. The difference from the literature this study is irrigation of the disc space with rifamycin directly without dilution. Treatment of spondylodiscitis was more costly and required further use of antibiotics. Our study results suggest that intraoperative rifamycin administration and postoperative three days use of peroral antibiotics are effective in the prevention of spondylodiscitis, obviating the need for more expensive therapies.
