Çocuk hastalıkları kliniği acil ve genel polikliniğine getirilen yenidoğan bebeklerin değerlendirilmesi

Özet
Amaç: Çocuk hastalıkları kliniği acil ve genel polikliniğine getirilen yenidoğan bebeklerin getiriliş nedenleri ve sonuçlarının değerlendirmesi. Yöntem: Gebelik yaşı 35 hafta ve üzerinde olan, yaşamın ilk 4 haftası içinde hastanemize getirilen bebeklere ait bilgiler poliklinik dosyaları ve hastanenin bilgisayar kayıtlarından retrospektif olarak incelendi. Bulgular: Bir yıllık sürede (1 Ocak - 31 Aralık 2005) 1063 yenidoğan çalışmaya alındı. Bebeklerin % 91.3’ü bir sorun nedeniyle getirilirken, % 8.7’si kontrol muayenesi için getirilmişti. Getirilme şikâyetleri: sarılık (% 37.7), ağlama ve huzursuzluk (% 14.2), öksürük-hırıltı-zor nefes alma (% 7.3), emmeme (% 6.2), kusma (% 5.1), göbekte lekelenme tarzında kanama (% 4.7), ateş (% 3.4), eritem-pişik-soyulma gibi cilt sorunları (% 1.9), ishal (% 1.7), morarmaydı (% 1.2). Tanılar; % 29.6 (n:315) yenidoğan sarılığı, % 15.8 (n:168) sağlıklı bebek, % 11.4 (n:121) beslenme bozukluğu-yetersiz beslenme, % 8.4 (n:89) hiperbilirubinemi, % 7.7 (n:82) infantil kolik, % 5.5 (n:59) sepsis ve sepsis şüphesi, % 4.3 (n:46) viral üst solunum yolu infeksiyonu, % 3.5 (n:37) pnömoni, % 2.1 (n:22) cilt sorunları, % 1.9 (n:20) konjenital anomali, % 1.7 (n:18) doğum travması, % 1.6 (n:17) idrar yolu infeksiyonu ve % 6.5 (n:69) diğer tanılar şeklindeydi. Bebeklerin % 20’sinin (n:206) yatırılarak tedavisine karar verildi. Sonuç: Çalışmamızda yenidoğan döneminde en sık hastaneye getirilme nedenlerinin sarılık ve beslenme sorunları olduğu saptanmıştır. Hastaneye yatma oranının yüksek olması, bu dönemdeki morbiditenin yüksekliğini ve poliklinikte yenidoğan ile ilgili karar veren hekimin yenidoğanın normal ve anormal durumları konusunda, özellikle sarılık ve beslenme sorunlarına yaklaşımda tecrübeli olması gerektiğini göstermektedir
Abstract
Aim: The aim of this sudy was to evaluate the clinical problems and out-comes of the newborns who have been brought to the emergency department or polyclinics of the Pediatrics Department. Methods: Data was collected from hospital records. All infants who were born ?35 gestational weeks and have been brought to the hospital in their 28 days of life were analysed retrospectively. Results: During one-year period (01 January to 31 December 2005) 1063 newborns have been brought. Of the infants 91.3 % presented with acute or nonacute problems and remaining 8.7 % have been brought for a control examination. The proportion of the complaints were; neonatal icter 37.7 %, crying/irritability 14.2 %, respiratory difficulty/dispnea/coughing 7.3 %, feeding difficulties 6.2 %, vomiting 5.1 %, umbilical hemorrhage 4.7 %, fever 3.4 %, minor dermatological problems such as erytema or diaper dermatit 1.9 %, diarrhea 1.7 % and cyanosis 1.2 %. The final diagnoses were; neonatal icter 29.6 % (n:315), healthy newborn 15.8 % (n:168), feeding intolerance 11.4 % (n: 121), hyperbilirubinemia 8.4 % (n:89), colicum infantum 7.7 % (n: 82), neonatal septicemia 5.5 % (n: 59), viral upper respiratory tract infection 4.3 % (n: 46), pneumonia 3.5 % (n: 37), minor dermatological problems 2.1 % (n: 22), congenital anomalies 1.9 % (n: 20), birth trauma 1.7 % (n: 18), urinary tract infection 1.6 % (n:17) and others 6.5 % (n: 69). Among these babies, 20 % (n: 206) were admitted to the Neonatology Department. Conclusion: In conclusion, it was found that newborns presented to the pediatric emergeny department of the hospital most frequently due to neonatal icter and feeding problems. The high admission rate (20 %) shows the severity of the morbidity in the neonatal period. The physicians and nurses who practice at the emergency department of the hospitals should have appropriate training and experience about management of the newborns
