Özet

Toksoplazmosis, zorunlu hücre içi protozoon olan Toxoplazmagondiitarafındanoluşturulan,dünyanınher bölgesindeyaygınolarakbulunabilenbirenfeksiyondur. İnsanlardaenfeksiyonanedenolabilenbuparazitozunen sık görülen klinik şekli yaygın veya bölgesel lenfadenopatidir. Ancak parotis kitlesi şeklinde ortaya çıkan toksoplazmosis literatürde az sayıda bildirilmiştir.Atmışyediyaşındabirkadın,üçaydırolan parotis bölgesindeki ağrısız kitle nedeniyle kliniğimize başvurdu. Fizik muayenede sol parotis bezi kuyruğunda 2x2 cm boyutlarında, sert, hareketli , ağrısız kitle tespit edildi. Parotis USG’de saptanan kitlenin parotis bezi ile ilişkisi net değerlendirilemeyince kontrastlı boyun MR istendi.MR’da 1,6 cm kitle lenf nodu olarak değerlendirildi.İstenen İİAB sonucunda lenfoma ekartasyonu yapılamayınca ,parotis neoplazmı tanısı ile yüzeyel parotidektomi uygulandı. Histopatolojik inceleme sonucu, toksoplazma lenfadeniti tanısını desteklemekteydi. Retrospektif çalışılan spesifik serum immünoglobulin değerleri de toksoplazmosis tanısını doğruladı ve 3 aydan daha önce enfeksiyonun geçirildiğini,akut bir enfeksiyon olmadığını gösterdi.Hastaya postoperatif üç hafta süreyle rovamycinetedavisiuygulandı.5aylıktakibindehastalık nüksüne rastlanmadı.

Abstract

Toxoplasmosis is a widely present ,mandatory ıntracellular protozoan infection caused by coccidian Toxoplasma gondii. Toxoplasmosis is a worldwide parasitic disease. Toxoplasma gondii, causes the infectioninhumans,withlymphadenopathyasthemost frequent clinical finding. Toxoplasmic lymphadenitis of the parotid gland are extremely rare. A 67-year-old womanpresentedwithapainless,parotidmasspresent for three months. Physical examination revealed a firm,2x2cm,mobile,roundmassattheleftparotidtail. Magnetic Resonance localized the 1.6 cm mass inferior of the left parotid gland (Fig. 1A). A fineneedle aspiration biopsy was non-diagnostic. An excisional biopsy taking in superficial parotidectomy was performed for a definitive diagnosis. The pathology examination revealed findings suggestive of Toxoplasmosis. These results confirmed a previous toxoplasmosis infection (IgG positivity), but not a currentinfection(IgMnegativity).Patientwasgiven3 weeks rovamycin theraphy.At the 5-month follow-up, thepatientremainedasymptomaticandnonewmasses developed in the neck.
