Özet

Post-travmatik pulmoner psödokist (P-TPP) akciğerlerinnadir görülen kaviter hastalıklarından biridir. P-TPPakciğer parankiminde gerçek epitel bulundurmayan içihava/sıvı ile dolu kaviter lezyonlardır. P-TPP genelliklekünt toraks travması sonrası gelişir. Burada, trafik kazasısonrası künt göğüs travması ile kliniğimize başvuran veP-TPP saptanan bir olgu sunulmaktadır. Travmadan 24saat sonra çekilen akciğer grafisinde sol alt zonda kistbenzeri lezyon olduğu görüldü. Kontrol toraksbilgisayarlı tomografisinde (BT) kistik lezyonunboyutlarında ve duvar gerginliklerinde artma vehemoptizi miktarında artma ve nefes darlığındakötüleşme görülmesi üzerine cerrahi girişim kararıalındı. TPP'lerin büyük bir kısmı spesifik bir tedaviyegerek göstermeksizin kendini sınırlayan iyi huylulezyonlardır. P-TPP, genellikle iyi seyirli olmasınarağmen hemoptizi ve sekonder enfeksiyon ile ilişkilikomplikasyonlar gelişebilir. Genellikle 2-4 ay içindespontan rezolüsyon gösterirler. Ancak çok azı, infekteolursa abse formasyonu oluşturabilir veya progresifolarak ekspanse olursa tansiyon kisti geliştirebilir. Bu türkomplikasyonlarda ise seçilecek tedavi, cerrahidir.Ayrıca izlemde progresyon gösteren psödokistlerintedavisinde cerrahi seçenek daima hatırlanmalıdır

Abstract

Post-traumatic pulmonary pseudocyst (P-TPP) is anuncommon cavitary lesion of the lung. P-TPP is anair/fluid filled cavity without a true epithelial liningwithin the pulmonary parenchyma. P-TPP usuallyoccurs following blunt chest trauma. Here, we report acase of P-TPP which occurred after blunt chest traumain a traffic accident. The chest X-ray 24 hours afterthe accident revealed cyst like lesion on the left lowerzone. Due to increase in cyst dimensions and walltension, which was seen on the follow-up thoraxtomography, and an increase in the amount ofhemoptysis and worsening shortness of breath,surgical intervention was decided for this case. Themajority of TPP’s are benign, self-limiting lesionswhich do not require specific treatment. Although,TPP’s are usually benign in nature, complicationsassociated with hemoptysis and secondary infectionmay develop. Usually, spontaneous resolution is seenin 2ı4 months after trauma, but a few of them formabscess or tension cyst after progressive expansionand in these circumstances surgical resection isindicated. Also, the surgery option should always beremembered for patients who show progression duringthe follow-up
