Özet
İn-vitro olarak antibiyotik duyarlılık testleri, bakteriyel patojenin bir antibiyotiğin tedavisi sırasında ulaşılan in-vivo düzeylerine duyarlı olup olmadığının değerlendirilmesi amacıyla uygulanmaktadır. Rutin duyarlılık testlerinde bazı direnç mekanizmaları gözden kaçabilmekte; bu gibi durumlarda bazı ek testler gerekebilmektedir. Uygulanan ek testler sayesinde antibiyotiğe karşı oluşmuş direnç mekanizması tahmin edilebilir. Bu testler ayrıca direnç mekanizmasından etkilenen diğer antibiyotiklerin duyarlılıklarını saptamada yol gösterir. Duyarlılık testlerinin standart bir yöntemle uygulanması, sonuçlarının yorumlanması ve uygun raporlanması ile tedavi başarısızlıkları en aza indirilebilmektedir. Bu derlemede Gram negatif bakterilerde genişlemiş spektrumlu beta-laktamaz tarama ve tanı testleri ile bildirimleri, AmpC beta-laktamaz tanı testleri ve karbapenemazların tarama, tanı testleri ve bildirimi üzerine güncel bilgiler sunulmuştur
Abstract
In vitro antibiotic susceptibility testing is applied in order to determine whether the bacterial pathogens are sensitive to the in vivo levels that were achieved during treatment. In routine susceptibility tests, some resistance mechanisms may not be noticed; some additional tests may be needed in such cases. The resistance mechanisms can be estimated through additional tests. These tests indicate how to determine the sensitivity of the other antibiotic that are affected by the resistance mechanisms. Treatment failures can be reduced by a standard method implementation of the susceptibility tests, interpretation and appropriate reporting of the results. The interpretation and reporting of the antibiotic susceptibility results is the task of the clinical microbiology specialist who made the tests. In this review, current information about the screening, detection and reporting of extended-spectrum beta-lactamases, AmpC beta-lactamases and capbapenemases in Gram negative bacteria are presented.
