Özet

Amaç: Bu çalışmanın amacı, total abdominal histerektomi ve bilateral salpingoooferektomi (TAH BSO) operasyonlarında visseral ve parietal peritonun kapatılmamasının postoperatif klinik süreçteki etkisini araştırmaktır. Yöntem: Mart 2001 - Ocak 2004 tarihleri arasında, bir tersiyer merkezde, benign endikasyonlar ile uygulanmış 150 ekstrafasiyal TAH BSO olgusu çalışmaya alındı. Olgular visseral ve parietal peritonları kapatılan (75 olgu) ve kapatılmayan (75 olgu) olarak iki gruba ayrıldı. Operasyon süresi, kullanılan analjezik madde tipi ve sıklığı, bağırsak fonksiyonları ve komplikasyon verileri hasta dosyalarından retrospektif olarak toplandı. Her iki grup farklılıklar bakımından karşılaştırıldı. Bulgular: Peritonları kapatılmayan grupta, postoperatif 1. günde 2' sinin (%2,7), 2. günde 18' nin (%24,0), 3. günde 73'ünün (%97,3) analjezik tedavi almadığı görüldü. Peritonları kapatılan grupta ise postoperatif üç günlük süreç içinde analjezik tedavi almayan hasta yoktu. Analjezik gereksinimi değerlendirildiğinde, peritonların kapatılmadığı grupta, analjezik gereksinimi, peritonların kapatıldığı gruptan istatistiksel anlamlı olarak daha düşüktü (p 0.05). Sonuç: TAH BSO operasyonlarında peritonların açık bırakılması, postoperatif süreçte komplikasyon sıklığını artırmadan analjezik ihtiyacını azaltmaktadır. Conclusion: Peritoneal non-closure in TAH BSO operations reduces the need for analgesics without increasing the frequency of complications in the postoperative period.

Abstract

Objective: The aim of this study was to investigate the effect of non-closure of visceral and parietal peritoneum on the postoperative clinical process in total abdominal hysterectomy with bilateral salpingo-oophorectomy (TAH BSO) operations. Methods: One hundred fifty extrafascial TAH BSO cases with benign indications in a tertiary center from March 2001 to January 2004 were included in the study. Cases were divided into two groups as those with visceral and parietal non-closure (75 cases) and and those with closure (75 cases). Data were collected in terms of operative time, analgesic requirement, bowel functions and complications retrospectively and the groups were compared for differences statistically. Results: Out of 75 patients with visceral and parietal peritoneal non-closure, 2 (2.7%) on day 1, 18 (24.0%) on day 2, 73 (97.3%) on day 3 did not receive analgesic treatment. In the group with visceral and parietal peritoneal closure, there were no patients who did not receive analgesic treatment during three days postoperatively. When analgesic requirement was evaluated, analgesic requirement was statistically lower in the group with peritoneal non-closure than the group with peritoneal closure (p 0.05).
